The program shall maintain on file for each staff member: (1) Documentation of the staff’s qualifications, including a resume, transcripts, diplomas, or any other documentation, which demonstrates their qualifications; (2) Documentation of a completed background check in accordance with He-C 4002.41; (3) A record of health screening, in accordance with (b) below; (4) Documentation of orientation, certifications, training, and professional development, as applicable; (5) Documentation verifying the staff reviewed the program’s emergency operations plan in accordance with He-C 4002.16(j); (6) Documentation verifying the staff reviewed their job description required in He-C 4002.16(e); and (7) Documentation of supervision regarding disciplinary actions or investigations specific to the staff member.

The licensing coordinators' review of five staff files revealed there was no documentation of qualifications on file for one new staff. Corrective action: New staff was asked to produce necessary paperwork for her file and now has all up to date items present. Going forward: new hires will be required to produce all necessary paperwork before starting work, I plan to use the staff matrix as a checklist during the hiring process.

Child care staff shall: (1) Teach children the importance of hand washing with liquid soap and running water; and (2) Instruct, encourage, remind, or assist infants and children as needed throughout each day to wash their hands as necessary to comply with (a)(1) through (5) above.

The licensing coordinator's observation that the staff in the infant room staff did not assist a child with washing their hands after their diaper change. Corrective action: Clean diaper practices poster was posted in infant room to remind everyone the correct way to change diapers.

He-C 4002.30(b) · Correction status not published

Except for nature-based programs that operate solely outside, programs shall conduct fire drills in accordance with the following: (1) Programs that operate continuously throughout the year shall conduct at least one drill each month of operation; (2) Programs that operate only during the school year shall conduct a drill within 14 days of opening each year and monthly thereafter; (3) Programs that operate only during the summer months shall conduct one fire drill in each month of operation; (4) Child care personnel shall conduct fire drills at varying times during operating hours, including night time hours, if applicable, to ensure that each child attending the program experiences fire drills; (5) Child care staff shall activate the actual fire alarm system for the building for at least 2 of the required monthly fire drills required each year and use a fire alarm or smoke detector to signal all other fire drills; (6) All children and child care staff shall evacuate the building during each fire drill; and (7) Child care staff shall check daily attendance records to ensure that all children and staff are accounted for, after the building is evacuated.

Two licensing coordinators review of program documents revealed that the program did not conduct fire drills in February, May, June, or July 2024. Corrective action: As director of the center I will make sure that we will conduct a fire drill every month along with other emergency preparedness drills. I will also make sure to record every drill on the fire drill log posted on the bulletin board for everyone to see.

He-C 4002.17(a) · Correction status not published

All center directors, agency administrators, site coordinators, or site directors, and all other child care staff who are responsible for the supervision of children, or who are necessary for the staff to child ratios, shall keep on file documentation of completion of a minimum of 6 hours of professional development, which shall be completed in accordance with the following: (1)  Within 90 days of the first date of employment; (2) Within 2 weeks for programs operating 3 months of the year or less; or (3) By providing documentation of previous completion.
