Action the program has taken or will take to correct the citation: Upon identifying the error during the review, I immediately logged into the staff roster to remove the staff member who departed on 11/25/2025. 2. Steps the program will take to ensure compliance in the future: To ensure all future departures are recorded within the required five business days, I have implemented a new exit protocol. This includes adding "Update Licensing Roster" to our formal employee termination checklist. Additionally, I will perform a monthly audit of the staff roster against our current payroll to verify that all active and inactive staff are correctly reflected. 3.
