Licensor reviewed three records and one had an out of date progress report. Please update and confirm once completed. For future purposes please take note of the following: *For school age children, the progress report must be prepared at least annually, at the midpoint of the child’s program year. *For toddlers and preschoolers, the progress report must be prepared every six months. *For infants and children with identified special needs the progress report must be prepared every three months.
Licensor observed an infant asleep in a swing with a blanket. In accordance with Safe Sleep violation protocol, please have parents sign the Safe Sleep Letter, sign the Safe Sleep Pledge and complete the Strong Start Safe Sleep training within one week of this report. Please provide documents and training certificate to EEC once obtained.
