An educator who has worked at the program for more than a year did not have documentation of current first aid training.
Hazards were found in the outdoor space, including tripping hazards on the stairs of the climbing structure and the slide exit area exceeded 11 inches above the protective surface.
It was found that the surfacing in the fall zones had an inadequate depth of impact absorbing material under and around the swings and climbing structures. It measured approximately 3 inches under the swings and 4 inches under the monkey bars and 3 inches near the dome climbing structure.
