Staff First Aid Training
Through a review of three staff files and the staff records checklist, the following was noted: 1. Six staff do not have current first aid certifications 2. One staff file was missing a staff form 3. One staff file was missing documentation of interview and references.
Through a review of seven student files, two were shown to have expired face sheets, transportation plans, and emergency first aid consents. The other five did not have dates on the first aid consent, face sheets, and transportation plans

It was observed during the visit that the program is providing care for a group of Preschool-aged children without the licensed capacity to do so. The prior licensee directed the program [REDACTED] to do so. Additionally, the program currently lacks proof that the staff has the proper credentials to care for Preschool children.
