Educator was grandfathered in for an older structure in back yard, but she has added a new play structure in the area. Furthermore, there is a climbing dome in the yard. A fall zone is now required for both structures. Please add shock absorbent materials (i.e: mulch) that is 9 inches deep and spans six feet out from the entire structure. There is one area of the structure that is too close to the fence and cannot be used; please make it inaccessible. Confirm once completed and a follow up visit will be scheduled.

Safe Sleep
Educator had a perspective assistant present, [REDACTED] is in the process of becoming licensed but has not yet been approved. Please ensure only individuals with an approved background check are on the premises.
Licensor reviewed five records and two were missing up to date medical forms and enrollment packet. Please ensure these documents are updated yearly. Licensor emailed Educator the annual update sample form.
Educator had appropriate napping materials for children, however the blankets were not stored in a sanitary manner. Educator stored blankets on top of one another. Please create a new system to ensure none of the children's belongings are touching.
Licensor walked through indoor space and there was one hazard observed. In the play room, the heating baseboard was missing it's cover. There were sharp materials within this and a child was asleep right next to it. Please add this cover back on to ensure there are no injuries. Licensor walked through outdoor play space, which is located in the backyard. There were no hazards observed.

Attendance was not accurate; it was missing time in and out. Please ensure this is filled in daily and it is accurate at any given point of the day.
Educator had a prospective assistant, REDACTED]present during this visit. [REDACTED] was interacting with the children. Licensor informed Educator that Michelle cannot be present and helping with children until she is fully approved by EEC.
There were three exits, and two were missing signs. Please add signs and upload pictures once completed.
There was an 11 month old infant who was sleeping with a loose blanket. Licensor spoke with Educator and assistant at length about safe sleep policy. Please upload/email the Safe Sleep Pledge, the Safe Sleep Parent letter, and complete the Safe Sleep online training.
