The licensor reviewed one staff file with the following findings: documentation of interview, verification of references, staff bi-monthly observations, and annual staff evaluation are not on file; the EEC PQ Registry form on file expired 2/8/2025.
There is no evidence of a certified/qualified staff member assigned to be the designated in-charge administrator on site.
The licensor observed an electric blanket in the smaller classroom which poses a fire hazard; the wire/cord for the air purifier is accessible to children and poses a tripping hazard.
Licensor observed a plunger in the bathroom that is accessible to the children; First aid kit is stored on top of a half wall and is accessible to children.

At the time of the visit three child files were reviewed and the following was found: one file did not include a developmental history and needed an updated physical.
Two staff files were reviewed, and the following was found: both files needed updated physicals and BRC suitability letters printed and placed in the file.
The program had one child with with an emergency medical condition. The child's individual health care plan did not contain all EEC required information.

At the time of the visit the attendance was in the office instead of with the children in the classroom and out on the playground.
The licensor observed the emergency first aid kit in the outdoor space on the ground and accessible to children.

1 staff member on site does not have a BRC; licensor informed the licensee that the staff member must leave the premises immediately and cannot return to the program without a suitable BRC.
3 children's files were reviewed with the following findings: 3/3 children have expired physicals.
Licensor counted 17 children present, and the attendance log noted 19 children present.
Licensor observed 10 children napping in the small room next to the main classroom and noted the rest mats were too close together. Licensor referred to the room measurements on file which is 177 square feet and only allows for up to 5 children at one time per EEC Regulation requiring 35 square feet of indoor space per child.
Staff schedule is not current.
EEC Medical Consent for 1 child with a documented allergy is expired.
IHCP and Medical Action plan for 1 child with a documented allergy/medical condition are expired.
One child did not have all of their prescribed emergency medications onsite at the time of the visit.
