Indoor Space
1) One child's Individual Health Care Plan (IHC) included a medication dosage of 10 (MG) solid medication. The program was provided with liquid medication, and the conversion rate to ML (Liquid medication) is unknown at this time. 2) One child's IHC Plan included a medication dosage of 5 MG and the child's Medication Consent Form included a medication dosage of 5 ML. Medication dosages need to be consistent and relative to the type of medication received (MG solid or ML liquid).

In the Pre-K classroom, there was a toilet bowl plunger and a toilet bowl brush in a stall accessible to the children,
The Allergy/Medication posting did not include that a child had a medication for a mild allergic reaction.
Outside, the toddler group's first aid bag, which included medication, was stored in an accessible location for the children.

Three children's files were reviewed. 1) Three children's files did not include an arrival and departure transportation plan signed/date by a parent. 2) One child's Individual Health Care Plan did not include staff who were trained in the administration of the medication, who trained the staff, doctor and parent signature and date. 3) One child's Medication Consent Form was incomplete. 4) One child's enrollment forms (except for First Aid and Emergency Care Consent Form) were signed and dated over a year ago and have expired

The expiration date on a plastic container of pretzels reads 4/25.
2) In the Preschool Classroom, a metal apple slicer was found in a drawer accessible to the children. 2) In the Preschool Classroom one sink was clogged and the water was not draining properly.
The Allergy/Medical Condition List did not include one child's food allergen that was noted on the child's current physical.
The fire drill log was reviewed. The program did not conduct a monthly drill in 12/24 and 5/25. Evacuation drills must be conducted monthly.
One child's Individual Health Care Plan called for a medication to be administered for an allergic reaction. The parent has not provided that medication to the program.

In the Pre-K library area, there was a black rug where on edge of the rug was curled up causing a potential tripping hazard.
The Fire Drill Log did not include the Route Used to ensure that all exits including alternate exits were being used.
The evacuation crib does not fit through the first doorway leading to the front door exit that includes a gate.
The thermometer in the Pre-K refrigerator read 54F. Food requiring refrigeration should be stored between 32F-40F.
The program had a current 2-week rotating menu. However, several times within those 2 weeks a non-specific fruit was served. The program should include documentation of what specific food was offered to children each day.
One child's emergency medication and prescription label had expired.
