The cubby unit in the proposed infant/toddler classroom is not secured and poses a tipping hazard. A foam child-sized chair and love seat in the hallway between the fist floor bathrooms and the existing infant/toddler classroom are obstructing the exit route to the front door and were removed during the visit.

The licensor reviewed the files of 2 children with a documented allergy/medical condition with the following findings: 1/2 children have an expired EEC Medication Consent form; 2/2 children do not have a required EEC Medication Consent form for a prescribed OTC medication on file.
The licensor noted the following during a walkthrough of the licensed space: the toy kitchen outside the 1st floor preschool classroom door is not secured and poses a tipping hazard; the cubby unit and toy kitchen inside of the 1st floor preschool classroom are not secured and pose tipping hazards; the cubby unit in the 2nd floor smaller classroom #4 is not secured and poses a tipping hazard; the large 2nd floor preschool classroom #5 has the following tipping hazards: high book shelf, toy kitchen furniture, high shelf in the bathroom without a window, unsecured cubbies and the licensor observed a printer and laminator accessible to children.
This investigation inspection recorded no violations or advisories.
The licensor observed 3 drawers in the Infant/Toddler classroom containing plastic bags, teacher supplies, and tubes of cream that are accessible to children. The tubes of cream were moved during the visit; 2 device wires are accessible in the Infant/Toddler classroom and were moved during the visit; the latch bolt on the door marked "closet" near the entrance is not functioning properly and leads to an area currently undergoing construction which is accessible and poses a safety hazard to the children.
Equipment is clean, safe, secured, and poses no hazard.

There were no evacuations drills logged for November or December 2022.
At the time of the visit three child files were reviewed and the following was found: one child needed an updated physical and two files needed annual updates completed.

The licensor reviewed the files of 4 children with the following findings: 1 infant does not have a quarterly progress report on file for 9/2025.
The licensor reviewed 2 staff files with the following findings: 1/2 staff do not have evidence of written bi-monthly observations on file; 1/2 staff do not have documentation of interview on file.
Attendance for the Infant/Toddler classroom reflected 6 children signed in but only 4 children were present.
Prescribed medications for a child with a documented allergy/medical condition are expired.
The licensor observed an infant/toddler staff member change a child and not wash the child's hands, their own hands, or sanitize the changing pad.
The toy kitchen in the Infant/Toddler classroom is not secured and poses a tipping hazard.
The location of the first aid bags in the Infant/Toddler and Preschool classroom are not posted. The location of the Health Care Policy was posted during the visit.
The Infant/Toddler classroom first aid bag is missing and instant cold pack that was put in the bag during the visit; the preschool first aid bag is missing a gauze roll and a CPR/FA mask/shield.
The licensor reviewed the file of a child with a documented allergy/medical condition with the following findings: the EEC Medication Consent form for 1 child is expired.

One eight-year-old child was present in the program (child of a staff member); the program is not licensed for school age care and did not have any required documentation on file for that child.
Staff Schedule and Staff Record Checklist do not include an active program contact listed as the program director in the LEAD portal.
Licensor observed artificial grass ground cover in the outdoor licensed space; there are three play structures (one spring-based riding toy, one slide, one crawl tunnel) that require impact absorbing material in the fall zone of each piece of equipment; the program director advised that a small padded rectangular mat was placed on one side of the spring-based riding as a safety measure.
A staff member present during the visit does not have a suitable BRC for the program.
Licensor observed a basket containing creams/ointments in the bathroom that is accessible to children.
A medical Action Plan on file for one child with a documented allergy/medical condition indicates that two [REDACTED] are required but only one [REDACTED] is in the program; one prescribed OTC medication is not in the program for a child with a documented allergy/medical condition.
One child with emergency medications did not have consent forms completed for each medication.
