1. Staff records had no evidence of bi-monthly observations, 2 One staff was missing evidence of a physical examination and MMR immunizations. 3. One staff had an expired physical examination dated 3/23/21.
There was no evidence to indicate that parent conferences are being offered.

This child's file also did not include evidence of a current physical or current immunizations nor the statement including the physical or immunization records were on file at the time. 2. One child's enrollment form was incomplete; the file did not include a transportation form and did not include evidence of a current physical exam and immunizations, or statement that the current immunizations or physical were current at the child's school.
