Evacuation procedures posted at each exit
3 children's files were reviewed and the following was noted: 1. 1 progress report was not dated so it could not be determined when it was completed. 2. 1 child was missing evidence of an updated lead screening.
When observing in Classroom X the following was noted: 1. There were several large holes in the carpeted walls and smaller holes in regular walls posing a potential hazard. 2. Wooden shelving units were not secured and were potentially tippable posing a potential hazard.
Equipment is clean, safe, secured, and poses no hazard.
Supervision of children
Children were observed not washing their hands before and after water play in Classroom R
An Educator was not supervising children appropriately on the preschool playground. The Educator was observed seated on top of a picnic table outside on the playground for more than 20 minutes. The Educator would get up off the table for a minute and then return to sitting on it. This Educator would call children over to her instead of getting off of the table. The Licensor went outside and as soon as the Educator saw the Licensor she got up off the table, as soon as the Licensor went inside the Educator returned to sitting on the table. The Licensor brought to the Directors attention immediately and the Director went outside.
1 child's IHCP had expired 5/10/24.
When reviewing medication the following was noted: 1. A prescription for Albuterol stated to discard after 7/6/24. 2. A prescription for an epi-pen stated to discard after 5/14/24 and the medication expired on 7/26/24.
3 staff files did not have evidence of completing the EEC Strong Start required trainings.

An Educator in Classroom P did not follow the program's name to face attendance policy when transitioning children from the classroom to the hall, she only counted, she again did not follow it before entering the mud room to go outside as required.
When reviewing children's files the following was noted: 1. 1 child was missing evidence of a physical exam and lead screening. 2. 1 file was missing evidence of a valid off site consent and transportation form it was dated 4/2022. 3. 1 file was missing evidence of an off site consent and transportation form.
When observing in Classroom M the Licensor noted the maximum group size of 9 children was exceeded, there were 11 children in one group. All 11 children were signed in to one attendance sheet. This classroom is supposed to have 3 separate groups of 4 children in each group.
Attendance was not accurate in the Toddler O classroom; 9 children were present and only 7 children were signed in.
When observing in classrooms the following was noted: 1. The edges to area rugs in several rooms were rolled up posing a tripping hazard. 2. A speaker was on a high shelf with the cord running down the wall to the plug not secured in Classroom X. 3. Dramatic play sink inserts were missing from Classroom X and P
Screens to windows were torn and in need of repair in Classroom T.
When observing in classrooms the following was noted: 1. Tables were not washed and sanitized prior to the children eating lunch in Classroom V, the Educator only sanitized the table. 2. The diaper changing surface was not washed prior to disinfecting as required in Classroom A.
An Educator in Classroom D took both hands off of a child while the child was being changed on an elevated surface.

Peeling and chipping paint and holes with exposed plaster were found in multiple classrooms throughout the program.
