Children's Records
A review of all staff files showed missing documents such as ongoing trainings, medical records, copies of licenses, certifications.
After reviewing the children’s records, it was found two children with allergy that were not posted.
During the visit, it was observed that a kitchen play set was not attached to the wall, posing a tipping hazard. In addition, a blow dryer with an unraveled cord was found under the bathroom sink cabinet, posing a choking hazard.
During the visit, it was observed two PQ registrations were expired for two program assistants.

A review of staff records showed the licensee did not complete the required 30 hours of training for license renewal; please submit documentation for the remaining 8 hours of training for review. Additionally, four assistants had not consistently completed the required ongoing training for 2023, 2024, and 2025.
A review of the files showed one child’s progress report has not been completed.
A review of seven children’s records showed that the general permission form from the enrollment packet had not been fully completed for two children. Additionally, one child’s progress report was not completed.
During the visit, it was observed that the educator removed soiled diaper changing materials and then washed their hands in the kitchen sink.
Temperature read at 30.
During the visit, all the children were placed in the nap room, where they remained for approximately 2.5 to 3 hours. None of the children were sleeping; some were standing inside the cribs, and one child had been crying for over an hour. Both assistants were present in the room, but the children remained crying and agitated. The licensor provided technical assistance to the educator, advising the educator to check on the children and provide additional support to the staff. When asked about the curriculum, the educator was unable to provide a clear explanation and stated that the assistant could better explain, noting that they are not actively running the program. When spoken to, the assistant was also unable to clearly explain the curriculum or planned activities.
Reviewed progress reports for the seven children present. Five of the files are due for updated progress reports.

It was observed that within the two diapering tables there was access to baby wipes and diapering creams. It was observed that one of the diapering mats is showing wear and tear and needs to be replaced. It was observed that the power cord to the window ac within the play area was not secured and easily accessible to the children.

Attendance records for today's visit were reviewed. It was observed that there were 8 children present and only 4 children were signed in. Assistant signed in the other 4 children after the licensor asked who was present. It was also observed that there were 3 assistants present but only two were signed in.

Review of children’s files was completed. Files for children present were reviewed. It was observed there were two files that did not have an updated progress report.
There was a small television on top of the mini fridge that is easily moveable. There was access to vinyl gloves, diaper rash cream, baby powder, and baby wipes within diapering tables.
Educator did not have proof of ongoing professional development trainings for 2022. Educator had proof of 20 hours of ongoing professional development hours and needs an additional 10 hours to complete the required 30 hours of training

Attendance records were reviewed. Children and staff attendance records reflect time in and time out. Educators did not sign in for today’s attendance.

Evacuation logs were reviewed. For the month of October 2022 a drill was not documented; testing of the alarms was documented. Educator indicated due to covid closure in October drill was not practiced.
Review of children’s records reflected three children not having an updated physical on file including one that had no lead screening. Two files did not have general permission signed. One file did not have the annual update. Educator will be required to submit children’s records checklist.
It was observed there was a spray adhesive remover accessible under bathroom sink. There was access to plastic bags in two of the kitchen drawers.

Licensor observed a child pulling on the radio power cord which was accessible from the crib. Educator fixed during visit. Power cords were accessible to children.

Five assistant files were reviewed. It was observed that there were two assistants that did not have sufficient ongoing professional development hours and must have a plan to meet these requirements.

Licensor observed an expired (REDACTED). Educators stated they were aware of the expired (REDACTED) and had been addressed with parent. Expired medications must be disposed of appropriately and there should be proof of prescription.
It was observed that one of the diapering tables was easily tippable causing a safety hazard.
