Two staff files were reviewed. One file did not include a Verification of School Age Work Experience Form.
The following documents located in the library have expired. Building Inspection expired on 10/30/25, Fire Inspection expired on 1/24/26 and the Indoor & Outdoor IPM Plans expired on 3/19/26. Please upload current documents to the library for review.
One child with a noted (REDACTED) was not included on the Allergy List. The SC stated that this child no longer uses an inhaler. However, there was no documentation in the child's file indicating that the child no longer needed this medication.
(REDACTED) medication was not on the premises, therefore it was not available if needed. (REDACTED) has an IHCP for an (REDACTED) and and IHCP that includes the administration of (REDACTED). However, both medications were not on the premises. Therefore they were not available if needed.

The program uses the cafeteria only if the gym is in use. Evacuation procedures were not posted next to the exits in the cafeteria.
