Emergency Evacuation
The Licensee and an administrator did not have a current BRC. Both individuals were not at the program on the day of the visit.
Two medication consent forms were not current as required.
Two did not have a current individual health care plan signed and dated as required.
A review of staff records indicated the following: -One did not have documentation of having immunity to MMR's as required.
A review of evacuation drills indicated that the program did not perform an evacuation drill for the month of March 2026. Fire drills are to be held monthly with all groups of children.

A review of children's records indicated the following: -A child did not have a completed Individual Health Care Plan. -A child did not have a completed Medication Authorization Consent Form. -The child did not have documentation of having a complete physical exam before entering the childcare. -The child did not have documentation of having all immunizations as required.
A review of staff files indicated the following: -The licensee did not have a staff information form completed for an educator working at the program. -The licensee did not have documentation of an educator had a physical examination before working in the program. -The licensee did not have documentation of an educator having the required immunizations. -The licensee did not document an orientation was completed with a new employee. -The licensee did not have documented two reference checks for a new employee. -The licensee did not have documentation that an interview was completed before the employee was hired.

A review of nine children's records indicated the following: -One did not have evidence of a current physical examination. It expired on (REDACTED). -One did not have documentation of an Individual Education Plan(IEP).
A review of three staff records indicated the following: -One did not have evidence of completing the required EEC Strong Start Essentials trainings. -One did not documentation of having a complete physical examination prior to employment by a certified physician. -One did not have documentation of immunity to communicable diseases.
One educator did not have documentation of a current Background Record Check.
In classroom B the large covered trash barrel was in front of the door blocking the emergency exit.
The C Classroom did not have evacuation procedures posted. Diagrams of escape routes must be posted in all classrooms.
There were four water stained ceiling tiles in the preschool classroom. The Licensee must ensure that the physical facilities are safe, clean, in good repair, and free from hazards and clutter.
The evacuation exit from the school-age classroom was blocked by an easel. Exits and evacuation routes must be kept clear of obstructions.
The licensee did not document the effectiveness of each evacuation drill. The educator must hold practice evacuation drills with all groups of children at least monthly. The educators must document the date, time, exit route used, number of children evacuated and effectiveness of each drill.
The plastic workbench structure in the preschool classroom was not secure. The licensee must keep all equipment, materials, furnishings, toys, and games clean and in a safe, secure, and workable condition.
There was a metal picnic table broken into pieces and accessible to children in the playground. The outdoor play area must be free from hazards including but not limited to: a busy street, a parking lot, poisonous plants, water hazards, debris, broken glass, chipping, peeling or flaking paint, dangerous machinery or tools, and weather related and environmental hazards or small objects that could present a choking hazard to young children.
Three children did not have documentation of who is trained to administer the medication and who trained them to administer the medication. The licensee must maintain as part of a child’s record, an individual health care plan for each child with a chronic medical condition, which has been diagnosed by a licensed health care practitioner. The plan must describe the chronic condition, its symptoms, any medical treatment that may be necessary while the child is in care, the potential side effects of that treatment, and the potential consequences to the child’s health if the treatment is not administered.
Four injury reports were not documented in the Injury/Incident Log. The four injury reports were in the children's records only. The licensee must maintain a record of any unusual or serious incidents including but not limited to behavioral incidents, injuries, property destruction or emergencies. These reports must be reviewed by the licensee or Program Administrator on a monthly basis.
