During the visit, three children's files were reviewed. One file did not include a current Transportation Form. One file did not include a current First Aid Emergency Medical Consent Form. Two files did not include a current physical. One file did not include a Lead test date.
During the visit, two staff files were reviewed. One file did not include verification of two references.
In the Infant 2 classroom, a sippy cup was not labeled with the child's name.
There was an inadequate depth of impacting absorbent material under and around the falls zones of the climbing structures on the toddler and preschool playgrounds. Three wooden railroad ties along the walkway to the playground were splintering and accessible to the children.
A current Staff Record Checklist has not been uploaded to the Document Library as part of the renewal process.
