Open packages of Veggie Sticks, Rice Cakes and crackers were observed in cabinets in the Toddler 1 and Preschool 1 classrooms. Open packages of food must be stored in covered containers.

During the visit, three children's files were reviewed. One file did not include a current Transportation Form. One file did not include a current First Aid Emergency Medical Consent Form. Two files did not include a current physical. One file did not include a Lead test date.
During the visit, two staff files were reviewed. One file did not include verification of two references.
In the Infant 2 classroom, a sippy cup was not labeled with the child's name.
There was an inadequate depth of impacting absorbent material under and around the falls zones of the climbing structures on the toddler and preschool playgrounds. Three wooden railroad ties along the walkway to the playground were splintering and accessible to the children.
A current Staff Record Checklist has not been uploaded to the Document Library as part of the renewal process.
Evacuation procedures were not posted next to the exits in the Toddler 1 and Preschool 2 classrooms.

The preschool classroom did not have evacuation procedures posted at each exit in the classroom. All exits must have written procedures and diagrams for emergency evacuation.

The allergy list was not privacy protected. All children's allergy information must be privacy protected unless the parents/guardian have given the program written permission to post without privacy protection.
The epi-pens were stored in a drawer in the preschool classroom that was unlocked and accessible to children. The medications must be stored in a locked cabinet not accessible to children.
