Diapering and Toileting Procedures
On (REDACTED) the entire day. All other children's diapers were changed appropriately. This (REDACTED) . The educator was terminated from the program. The licensee now has multiple privacy protected listings of who wears diapers in the group posted. The licensee had a staff meeting last week to work on making sure all children are cared for appropriately.

A review of educator records indicated the following: -One did not complete their EEC orientation as required. -One did not register annually with the Department of Early Education and Care.
Multiple classrooms had ceiling tiles with water stains.

The old physical examination form expired on 10/31/24.

A review of eight educators records indicated the following: -One educator was on the staff schedule however was not documented on the staff records checklist. -Three did not register annually with the Department of Early Education and Care. -Two did not have documentation of two verbal reference verifications, including one professional or academic reference. -One did not have documentation of a current First Aid certificate. All educators must obtain within six months of employment, and must maintain thereafter current certification of training in basic first aid appropriate to the population served. -Five did not have documentation of a current statement signed by a physician certifying that they are in good physical and mental health.
The following are Safe Sleep violations: -One child was drinking a bottle while lying down in his crib. -One child had a bead bracelet attached to their binky while lying in a crib. -One child had an elastic attachment to their binky while lying in the crib. No Bottles. Bottles must never be propped, and babies should not suck on a bottle while sleeping. Propping the bottle increases the risk of choking and of ear infections. Falling asleep with milk pooled in the mouth leads to serious dental caries in developing teeth. • No Jewelry. Jewelry of any kind must be removed prior to placing a child to sleep. Necklaces, earrings, bracelets, and anklets, including those used to help with teething or those worn for cultural or aesthetic purposes are not safe and are prohibited for sleeping or resting children.

The IHCP specifies the child must have a up to date medication on site. -Two did not have documented who trained the educators to administer the medication. -Two did not have documentation of which educators were trained to administer the medication.
