A review of five children’s records showed that four physical were expired, and one child’s Individual Health Care Plan was missing.
During the visit, it was observed that the program has a child with a documented allergy; however, an Individual Health Care Plan was not on file.

During the visit, multiple safety concerns were observed. In the playroom, a play kitchen set was observed to be unsecured and not attached to the wall. In the kitchen, multiple medication bottles were observed on the counter and accessible to children. In the bathroom, acetone and hair powder were observed on the sink. Inside a drawer, wipes and gloves were accessible to children. No paper towels or other means for hand drying were observed. Electrical outlets located near the sink were observed to be uncovered. The licensor further observed an unlocked closet in the bathroom containing various items accessible to children, including but not limited to hairspray, hair products, alcohol, mouthwash, a blow dryer, and medication. In addition, the wall cabinet above the sink was observed to be missing its mirror/door, leaving the cabinet open and unsecured. The open cabinet contained multiple personal hygiene and toiletry items, including but not limited to creams, lotions, oral care products, grooming tools, and other small containers, all of which were within reach and accessible to children.
During the visit, the living room exit was observed to be blocked by multiple items, including but not limited to two strollers, two chairs, bicycles, and various toys.
The licensor observed the following hazards: classroom; one toy shelf not secure to the wall. In the bathroom: there was a razor blade on the edge of the bathtub, the trash did not have a cover, a cord for the water pick was accessible, there was a bottle of lotion on the sink counter, a bottle of mouthwash in the cabinet over the sink accessible because it could not close. In the kitchen there was an open bag of trash on the floor and there was a fan plugged in and on, on the floor accessible to the children and the cord was also accessible. In the living room the cords by the TV were accessible to the children

The licensor observed one slide that is above 18 inches that did not have the appropriate Fallzones surrounding it.
The educator did not take into consideration the ages of the children and the equipment used when she buckled 3 children ages 20m, 2y and 2y into infant seats/swings when she left the room briefly and was unable to maintain direct eye contact with them. The equipment used was not appropriate for the ages of the children.
Although the educator has completed 30 hours of training there is not 10 in diverse learners

A review of the records show that 1 child did not have pgs. 2&6 completed. 2children did not have general permissions complete 1 child needs an updated medical and 2 children need an annual update and 1 child the top parts of the pages were cut off
During today’s visit the educator stated that the (REDACTED) old infant arrives asleep in the car seat and she leaves her in there to complete her nap
The licensor observed the following during the visit: in the bathroom there was hairspray, hair products and mouthwash accessible to the children on the sink. There is shampoo accessible on the edge of the bathtub. In the kitchen there are several drawers that do not lock and contain hazards such as: a pizza cutter, a screwdriver, bleach and other cleaning supplies. There was also a mop and 2 vacuums accessible. In the living room there were several cords accessible and a pocketbook accessible on the couch

First Aid Kit
There were two children with noted allergies. The Educator was aware of the allergies, but did not have the required posting.
The first aid kit was missing the children’s emergency cards.
There were outlets in the living room that were not covered. There was a plug in air freshener in the living room that was accessible to the children. There were cabinets and drawers in the kitchen and in the bathroom with hazards accessible to the children (cleaning products, plastic bags, wine opener, etc.)
