Current consent for administered medications
During the visit the licensor observed, a group leader sitting at a table using their cellphone while a child was watching them. Educators must not engage in any other activities or tasks that could unnecessarily divert their attention from supervising the children.
The following documents located in the library have expired. Building Inspection expired on 2/18/26, Fire Inspection expired on 1/24/26 and the Indoor & Outdoor IPM Plans expired on 3/19/26. Please upload current documents to the library for review.
One child's Transportation Plan was not signed and dated by their parent.
At the time of the visit, the Fire Drill Log was reviewed. Only a practice drill for 9/16/25 was located in the log. Practice fire drills must be conducted monthly and documented. This was previously cited on 5/5/25.
The Individual Health Care Plans for four children with a chronic medical condition were not available for review during the visit. This was previously cited on 5/5/26.
The following was noted during the visit: four children's (REDACTED) medications stored at the program had expired.
One child's Individual Health Care Plan included the administration of (REDACTED). This medication was not on the premises, therefore it was not immediately available if needed.
Four children with medication on the premises did not have Medication Consent forms available for review during the visit. This was previously cited on 5/5/26.

The first aid kit did not include tape or an ice pack.
At the time of the visit, the Fire Drill Log was reviewed. Practice drills were not conducted during the months of March and April of 2025.

One child's epi-pen was stored in the nurse's office. The nurse's office is not open during before school hours. Therefore, the medication was not immediately available if needed.

Upon review of staff records checklist the following was found: Staff #1: no proof of Essentials training completion, no PQ registry noted, no professional development logged, and in need of a current physical Staff #2: in need of a current physical Staff #3: does not have evidence of first aid and CPR certification
