During the visit two staff files were reviewed. One file did not include a current physical (last physical dated 2022).
During the visit, a garbage barrel was observed in the hallway without a cover.
A Preschool child's Individual Health Care Plan (IHCP) was dated 3/31/25 and had expired. IHCPs must be updated annually.
During the visit, it was noted that one child's Medication Consent Form was dated 4/3/25 and had expired. These forms must be updated annually.
During the visit, a teacher in the preschool classroom brought a child to the bathroom outside of the classroom. This left the other teacher alone with 12 children. Staff to child rations must be maintained at all times.
During the visit, three staff files were reviewed. One file did not include verification of two references.

At the time of the visit two children with emergency medical conditions were reviewed and the following was found: some of the EEC required information was not complete on the IHCP forms.

In the children's bathrooms, wipes and plastic bags were accessible to the children.
During the visit, the following hazards were observed outdoors: white wood planks and a grill were accessible to the children. Six little tykes slides did not have the required 6 feet fall zones between each slide.

During the visit, eight staff files were reviewed. One file did not include a Staff Form. Three files did not include verification of two references. Four files did not include a current PQ Registry. Five files did not include a current physical. One file did not include evidence of MMR vaccinations. Two files did not include evidence of Program Orientation. In the School Age program, observations of staff working with children during the 2018-2019 school year were not available for review.
In the Preschool classroom, a child's [REDACTED] medication was not taken with the child to the playground. Emergency medications must be immediately available is needed.
In the Pre-K classroom, the lower walls near the rear exit must be repaired. There is a sharp edge on the door jam in the Gym and the threshold is in need of repair. There is rust on the wall vents and stalls in the boy's bathroom. The two couches located in the School Age program are not in good repair and must be repaired or removed.
In the School Age program, children were observed eating snack on the couches while placing their packages of food and drinks on the floor. Children must not be involved in other activities while eating snack.The licensee must provide space sufficient for children to eat in an uncrowded manner and to meet the needs of all children.
The garbage barrel in the Pre-K classroom does not include a cover as required.
The Allergy/Emergency Medication list posted in the School Age program is not accurate.
In the Preschool and School Age programs, eating surfaces were not washed and disinfected before and after each use.
The Preschool program is not providing the children with the opportunity to brush their teeth while in care. Educators must assist children in brushing their teeth whenever they are in care for more than four hours or whenever they consume a meal while in care.
Three children's Individual Health Care Plans did not include all of the required information.
The program's building inspection and fire inspection have expired.
