One open electric outlet was observed in the eating area.
During the inspection of the outdoor play space, the licensor observed at least three pieces of playground equipment with fall heights of 28, 32, and 29 inches did not meet the required fall zone of at least 6 feet in all directions from their perimeters. The playground equipment was positioned very close to each other, which does not comply with spacing requirements.
One child with prescribed life-saving was not listed on the Allergies and Life-saving Medications form.

Playground Safety - Fallzones
Licensor observed two playground equipment sets with a fall height of more than eighteen inches that did not meet the required fall zone of a minimum of 6 feet in all directions from their perimeters. For each piece of equipment, the fall zone must extend a minimum of 6 feet in all directions from the equipment's perimeter. Additionally, the wood chips covering the outdoor play area were compacted and did not meet the appropriate depth required by the EEC for impact-absorbing material. All playground equipment must be located within use zones that are covered with an adequate depth of an impact-absorbing material, in accordance with EEC policy.
During the inspection of the exit routes, Licensor observed a couch obstructing the exit that leads to the front of the house.

The Educator has 4 play structures. One structure is too close to the others which doesn’t allow for the 6 feet clearance on all sides, as required. Also, observed outside was the lawn mower which was accessible. Inside the home, the Licensor observed many loose cords in the corner near the desk. One shelf in the playroom isn't bolted and could pose a tipping hazard. The play kitchen isn't bolted and could pose a tipping hazard. Diapering creams and lotions were accessible near the diapering table.

Three outlets in the basement level are uncovered and accessible.
One child who comes in the middle of the day is being documented on the attendance as arriving in the morning. The attendance records don’t reflect the actual arrival time, as required.
EEC Assistant
Five of the children’s progress reports were reviewed, provider reported there are two children who require an updated progress report.
The provider did not have a barricade at the bottom of the deck stairway.
The basement exit directly leading outside did not have an exit sign posted.
The cabinet under the sink in the bathroom was observed to have a broken safety lock allowing access to disinfecting wipes, plastic bags and cleaning supplies. The baseboard heaters in the basement space are rusted and the one in the basement kitchen is broken. In the kitchen, there were cleaning materials on the floor next to the door along with an electrical cord. Two of the drawers were unlocked allowing access to a pair of scissors and a sharp knife.

In the outdoor play space, a lawnmower was accessible a long with sink and broken fencing on top of it.
Toothbrushes were stored inside zip lock bags inside the medicine cabinet.
Diapering material is ripped and educator is not using barrier to change the children.
Three crib mattresses have been covered with plastic material. One crib has plastic material being secured with safety pins which are hazardous if they come undone.
There is a swing set, large climber and slide that doesn’t meet current fall zone requirements. These play structures can’t be used with child care children.
There is one bookshelf in the playroom that isn’t bolted and poses a tipping hazard.
