Children’s emergency cards in first aid kit were not up to date.
Assistant file did not have an updated physical.
It was observed that there was access to a climbing structure (slide) that does not meet requirements for fall zones and ground surfacing. It was observed there was access to trampoline due to a chair being setup by the opening and mesh was opened. There were two uncovered grills.

Attendance logs were reviewed. It was observed there was one day in Jan 2023, one day in Feb. 2023, and one day in March 2023 where some children were not signed out. Attendance records must reflect daily arrival and departure times. Educator is not documenting attendance for her certified assistant.
Outdoor space was observed to have some construction taking place. There were hazards accessible within outdoor space. Hazards observed were multiple ladders, side paneling, trash was accessible, some wooden boards, a wheel barrow, electrical cord, uncovered grills, wire fencing at the bottom of the shed was observed to be bent leaving sharp ends accessible, there was a broken basketball net with sharp edges accessible, a lawnmower was accessible, on the deck there was access to large plastic bags, multiple boxes were stacked, and wooden boards were accessible.

The educator’s CPR expired on 10/17/2019. The First Aid that the educator had on hand expires on 11/2019, which the educator stated she has retaken but did not have proof.
The stairway does not have railings, as younger children need the support to be able to go up and down the stairs safely.
The educator is currently not utilizing her backyard as they are working on her deck and there are hazards accessible. She is currently taking children out for neighborhood walks. It was also observed that there was access to the swing set, which is not approved for use.
There were three emergency cards in the first aid kit that were not up to date.

First Aid Kit
Certified assistant Maria Rosa Hernandez’s file was reviewed. Assistant file did not have current physical, assistant’s CPR and First Aid certification is not valid; this is an online only certification which is not approved. Assistant has only completed 5 hours of her required ongoing professional development trainings. Assistant is required to complete 10 hours per year. Assistant can not be alone with children until in person CPR and First Aid is completed.
There were non-latex gloves and hand sanitizer observed to be accessible from the back exit. There was a can of disinfectant spray accessible at the top of the stairs leading to basement level. There were plastic bags accessible in the basement kitchen area.
Children were observed to be in space that has not been approved. The children were observed to be in the basement level, which the educator has requested for this space to be approved. The basement level was inspected today and will be approved, however it was discussed with the educator that she cannot utilize new space with the daycare children until it has been approved.
