The nap mats were not stored in a way that prevents them from touching one another
The white dress up shelf is not secure to the wall, the stairs leading to the upstairs exit are not clear. Outside there is a water table and a seesaw within 6 ft of the swingset.

The Educator had 9 inches of ground surface covering. However there were other toys/equipment within 6ft of the swingset that need to be moved.
Handwashing
The educator installed a swingset and the area around it does not the required 9in of ground surface covering and additional ground surface covering needs to be added at the bottom of the slide. There were 2 infant swings that were above 18 inches and did not have the required fall zone of 6ft clearance around them. The swingset cannot be used until it comes into compliance.

A ten month old infant was observed to be asleep in the pack and play with a small blanket attached to their pacifier.

The licensor observed the following: a wire running from the wall into a pack and play, wires accessible to the children in the playroom, the sink under the kitchen was not locked and cleaning supplies were accessible
The 5 month old infant was not being held while bottle fed. The regular assistant picked up the infant and held him at the licensors request
