Children’s files were reviewed. One file needs the annual update of the enrollment packet. Two files had a signed general permissions without any information on where children are taken. It was discussed with the educator since she does not go off premises that should be indicated on the general permission. Two files had missing physical description on the first page of enrollment packet. One file had no lead screening results and had a missing parent signature and signature date on page 5 of enrollment packet.

Three of the four children are not yet due for a progress report. The one child that is due, child’s file did not have a copy of recent progress report on file.
Children’s files were reviewed.There were 3 files that had expired or incomplete physical and one had no physical.
It was observed that there was access to standing water in a plastic gallon can. Se observó que había acceso a agua estancada en un bote de galón de plástico.
It was observed that the gate installed to prevent access to the living room area was not secured properly. This allows children access to the living room area where a large mirror was observed to be leaning up against a box which was easily tippable. This also gives children access to the stairway. Se observó que la barrera instalada para evitar el acceso a la sala de estar no estaba asegurada adecuadamente. It was observed that there was one child napping on a sleeping mat which was positioned in front of one of the emergency exits. Se hablo con la educadora en como organizar su espacio y utilizar su cocina para dar mas espacio libre a los ninos.
Evacuation drills from July-November of 2021 were not documented and evacuation drill for July of 2022 was not documented. Evacuation drill was observed during today’s visit. Se observo el record de prácticas de evacuación. Se observo que entre Julio-Noviembre del 2021 las practicas no se documentaron y la practica de Julio 2022 no se documento. Durante la visita de hoy se observo la practica de evacuación.
Ongoing Professional Development
It was observed there was a pile of hazards in the backyard that were accessible to children. Some of the hazards included wood panels, trash bags, paint cans, and other items that were stacked and couldn’t be identified. Educator indicated this is from work that is being completed for the educator’s living room. There was access to a ladder, larger wood boards, and the water hose was extended across the yard posing a tripping hazard. It was observed that throughout the visit (REDACTED) in care was in a bouncy chair entire time children were outside and when inside was placed in a high chair before educator took (REDACTED) out to bottle feed (REDACTED). It was discussed that the educator needs to give (REDACTED) opportunities to be on the floor and be given opportunity to explore freely and not be confined.
There are hazards accessible within the exit route from the outdoor space. Some of the hazards were a ladder, paint cans, and wooden panels. One child did not have a physical on file. Un expediente no tenia un físico.
Attendance records were reviewed. It was observed that there were four days between April and May of 2022 in which some children were not signed out. Los records de asistencia se revisaron. Se observo que en 4 ocasiones entre Abril y Mayo del 2022 algunos niños no firmaron la hora que se recogieron.
The educator has variety of developmentally appropriate materials within her indoor space. It was observed that there was not a variety of developmentally appropriate materials within the outdoor space for the children currently enrolled. La educadora tiene una variedad de materiales apropiados para el desarrollo dentro de su espacio interior. Se observó que no había una variedad de materiales apropiados para el desarrollo dentro del espacio al aire libre para los niños actualmente inscritos.
The educator has a written notification regarding firearms in the home, however there was not a copy of signed notification within children’s files. First aid kit did no have treatment tape, gauze roller bandage, non-latex gloves, and instant cold pack. Additionally the educator did not have her first aid kit with her while she was outside with the children. It was observed that there was a small mirror against the wall in the bathroom, which was easily tippable. Sharp knives were observed to be accessible in one of the kitchen drawers. The educator is redoing her living room, which work materials were accessible due to children having access to the dinning room. Emergency list did not have educator’s contact information, did not have poison control, backup persons were not labeled, and emergency number; (911) was not listed.
