In the Infant 1 Classroom, an infant was observed drinking a bottle while sitting in a baby bouncer. Infants must be held while fed a bottle. If the child can hold their own bottle, they must be placed sitting upright in a high chair.

Three children's files were reviewed. All three files did not include a current physical. Each child's file must include an annual physical.
During the visit, the attendance was inaccurate in the Infant 1 Classroom. There were eight children observed in the classroom but only six children were signed in on the attendance form.

A review of staff meetings indicated the following: -There was no documentation of the times of the staff meetings for the months of March and May 2023.
A review of emergency evacuation drills indicated the following: -There was no documentation of fire drills completed in the months of June 2023 and September 2023.
Ajax liquid soap was out on the children's bathroom sink. It has documentation on it that states keep out of reach of children. It was accessible to children.

A review of the classrooms indicated the following: -In Classroom "D" there were several ceiling tiles that had water stains on them. -In Classroom 169 there were broken floor tiles near the circle time rug. -In Classroom 174 there was a missing floor tile near the bathroom.
Classroom 159 was missing an emergency exit procedure. Exit procedures must be posted at every exit.
A review of medications indicated the following: -One had an expired (REDACTED).

A review of attendance records in Classroom 182 indicated that there were six children present. There were seven children present. A child had arrived within 10 minutes and educators did not document his presence.

A review of classroom attendance documentation indicated the following: -Room 163 had 8 children present, however, the attendance indicated 7 children present. Licensee must maintain accurate attendance records.
A review of the Staff Meeting Agenda's indicated the following: -Two staff meetings(10/20 and 11/20) did not have documentation of the length of the staff meeting. Staff meetings are required to be held every month for two hours.

On the preschool playground, a piece of the climbing structure was missing which left 2 screws exposed and accessible to the children.
During the visit a child with two Individual Health Care Plans were reviewed. Both plans were signed and dated by the child's physician on (REDACTED) and had expired. Individualized health care plans shall be valid for one year, unless withdrawn sooner, and must be renewed annually and following any change to the child’s condition for administration of medication and/or treatment to continue.
In the Toddler Classroom (Rm. 163), bibs used by the children were observed in the handwashing sink. Facilities used for hand washing after diapering or toileting must be separate from facilities and areas used for food preparation and food service.
During the visit, the SAC Transportation Attendance Logs were reviewed. For the O'Donnell Middle School (OMS), on 9/4/24, 7 children were not signed off the bus. On 9/5/24, 9 children were not signed off the bus. For the Hansen School, 10 children were not signed off the bus on 9/4 and 9/5/2024. During the week of 10/21-25 children were not consistently signed off the bus. When children are absent and not transported to the program, they must be marked as absent and the program must follow Transportation procedures. The Program shall note on the transportation attendance sheet the absence or late arrival of the child on the appropriate day and, if known, the name of the person who notified the program of the absence or the late arrival.
A review of medications at the program indicated the following: -Two did not have current medication consent forms for their prescribed medications. -One did not have a current Individual Health Care Plan. -Two did not have separate medication authorization forms for each medication to be administered. -One did not have documented potential consequences of medication not administered.

In the toddler bathroom the disinfecting spray bottle was accessible to children. All toxic substances must be inaccessible to children.
A review of medications indicated the following: -Two Individual Health Care Plan did not have documentation of what would happen if the medication was not administered.
