At the time of the visit, it was observed that the thermometer was not working and needed new batteries.

During the visit the Licensor identified multiple missing and expired records, including: seven expired enrollment packets, three expired progress reports, one missing progress report, one missing physical, and one expired physical.
The Licensor also noted that four children enrolled in care have documented (REDACTED); however, these allergies were not posted.

During the visit, it was observed three progress reports were expired and one progress report was missing.

At the time of the visit, the Licensor observed a knife on top of the kitchen counter, a can opener stored in a kitchen drawer, and the cabinets did not have any child safety lock. The outlets in the nap room were not covered, and the outlets in the bathroom were also not covered. In the nap room, there was a sewing machine accessible to children, along with scissors and pins.
The Educator’s parent handbook is missing some of the required EEC information
The last documented fire drill was in August of this year

Annual Registration in Professional Qualifications Registry (PQR)
(REDACTED) regular assistant. File was reviewed; assistant did not have proof of her ongoing professional development training; assistant must complete at least 10 hours for 2021 year.
Educator’s CPR and First Aid expired on 10/14/2021. Educator indicated both have been renewed and is waiting for cards to come in; once they are received educator will submit proof of renewal.

Care of young children - curriculum
When the licensor arrived all 3 infants were placed in infant seats. 2 were in car seats, one [CHILD] strapped in and one [CHILD] not strapped in. The third infant, [REDACTED], was in a swing, then placed down in a pack and play for nap. The other 2 infants were either held or kept in the car seat. The licensor did not observe the infants to be placed on the floor to explore their surroundings. Additionally the infants were not given any toys to use while they were in the swings/seats. This does not allow the infatns opportunities to move freely and achieve mastery of their bodies.

The educator’s and assistant’s PQ registry are both expired. Educator indicated having difficulty with PQ system; link to help desk will be sent.
