Ratios, Group Size and Supervision of Children
Six staff files for the school age programs were reviewed at the main office. The following was noted: One file did not include a current physical. One file did not include evidence of immunity to Measles, Mumps and Rubella. One file did not include a Verification of School Age Work Experience Form.
SM's staff file did not include evidence of the completion of the Essentials 2.0 (12 hr.) Training package.

Medication
The maximum group size for school-age children is 26 with 2 staff. During the visit, 65 children were observed in the cafeteria together. The children were not in assigned groups with designated group leaders. The program must utilize the gym as soon as possible to maintain compliance with group sizes. Groups larger than 26 children must be divided into smaller groups with designated staff supervising the groups to maintain staff-to-child ratios at all times.
