Two educators that had worked in the program over 6 months did not have First Aid Certification as required by EEC.
During the visit, two staff files were reviewed. One file did not include a current PQ registry. Two files did not include a current physical. One file did not include evidence of immunity to Measles, Mumps and Rubella.
Two educators listed on the Staff Record Checklist did not have a current Background Record Check (BRC).
One child's Individual Health Care Plan was signed by their physician on 2/15/25 and was not current.
One child's Medication Consent Form was expired.

Fall zones adequate depth of impact absorbing material
Bi-monthly observations of staff while working with the children were not completed.
The slide structure located on the toddler playground did not have impact absorbing material under and around the fall zones. All playground equipment must have a protective surfacing with the exception of any equipment that children use with their feet on the ground such as sand boxes, activity walls at ground level, and play houses.

A review of IHCP's indicated the following: -One did not have documentation of who trained the educators to administer the medication and which educators are trained to administer the medication.
This investigation inspection recorded no violations or advisories.
The licensee did not document on the fire drill log the effectiveness of each fire drill.

Medication
A review of staff records indicated the following: -One did not have documentation of two references. -One did not have documentation of completing the program orientation.
The licensee did not have a current Fire Inspection Letter from the local fire department.
This investigation inspection recorded no violations or advisories.
This monitoring outside differential licensing cycle visit inspection recorded no violations or advisories.
A (REDACTED) infant was observed sleeping in the crib fully swaddled. . Sleepers and sleep sacks that leave the infant’s arms free to move are good alternatives to blankets. Swaddling is prohibited for any child who can roll over. Swaddles may be allowed if recommended in a written order issued by the child’s health care provider and must allow the infant’s arms to move freely.
The older toddler classroom did not have emergency/evacuation procedures posted at the two exits.

The Building Inspection Certificate expired. Every program that is not located in a residence and every program that has a licensed capacity of more than ten children must provide a certificate of inspection from the local building department.
This renewal - monitoring visit inspection recorded no violations or advisories.
-In the infant, toddler, and pre-k classrooms there were stained ceiling tiles. The indoor space must be clean, safely maintained, well-ventilated and well-lit, of sufficient size for the children served, and must encourage play and learning.
A review of the injury/incident log indicated that one injury did not get documented in the injury/incident log. A copy of the signed injury report was in the child's record. The licensee must maintain a record of any unusual or serious incidents including but not limited to behavioral incidents, injuries, property destruction or emergencies. These reports must be reviewed by the licensee or Program Administrator on a monthly basis.
The licensee did not have a health care consultant with a current certification. Each program must have access to a Health Care Consultant who will be available to the program for consultation, as needed. Approve the program's health care plan at initial licensure and at each license renewal. Approve first aid training and training in medication administration for staff.
-A review of the medication stored @ the program indicated the following: -One did not have documentation of a current medication consent form for two medications.
In the preschool playground the large climbing structure did not have any impact absorbing material under the fall zones.

-The licensee did not have evidence of compliance with applicable fire codes. -The licensee did not have a date and signature on the Integrated Pest Management Plan.
A review of the preschool classroom indicated that there was peeling and flaking paint on the wall.
A review of eight children's records indicated the following: -Two did not have documentation of a parent signature on the First Aid/Emergency Medical Consent and Release Form. -Three did not have documentation of current Developmental History Forms. -Two did not have documentation of current First Aid/ Emergency Medical Consent and Release Forms. -Two did not have documentation of current Off Site Consent Forms. -Two did not have documentation of current Transportation Plans. -Two did not have evidence of an annual physical examination.
A review of two years of staff meeting documentation indicated the following: -Thirteen monthly meetings did not have documentation of the time the staff meeting ended. -Five monthly meetings did not have documentation of which educators attended the meeting. -The licensee did not have evidence of completing a staff meeting in September 2018.
A review of 5 staff records indicated the following: -Five did not have documentation of current CPR certification. -One did not have evidence of immunizations. -One did not have documentation of current First Aid Training. -One did not have evidence of registering annually with the Department of Early Education and Care. -One did not have evidence of two verified references.
-A review of the preschool classroom indicated that near the back door, the wooden shelf was not secure.
-The classrooms and children's bathrooms did not have covered trash cans. Garbage must be kept in lined and covered containers and all trash containers must be emptied at least daily.
The large climbing structure, the wooden enclosed area with the 4 equipment and the swings have no protective ground surface under them. Licensee was informed that they cannot use the equipment until an appropriate ground surface has been installed.
One Individual Health Care Plan (IHCP) did not have documentation regarding which educators were trained to administer the medication and who trained the educators to administer the medication. The licensee must maintain as part of a child’s record, an individual health care plan for each child with a chronic medical condition, which has been diagnosed by a licensed health care practitioner. The plan must describe the chronic condition, its symptoms, any medical treatment that may be necessary while the child is in care, the potential side effects of that treatment, and the potential consequences to the child’s health if the treatment is not administered.
