At the time of the visit three staff files were reviewed and the following was found: 1. Staff A was missing evidence of two references and did not have a current annual review. 2. Staff B was missing evidence of two references, did not have a staff information form, and did not have a current annual review. 3. Staff C was missing evidence of two references, did not have current first aid or CPR, and did not have evidence of MMR.

At the time of the visit two children with emergency medical conditions were reviewed and the following was found: one child was missing a portion of their prescribed medication.

During the visit, the Fire Drill Log was reviewed. The log did not include documentation showing that drills were conducted from January to June of 2023. Practice fire drills must be conducted monthly.

Two educators working in the program did not have a completed Background Record Check.
At the time of the visit, the Renewal Application Transaction had not been submitted in LEAD. The Renewal Application must be submitted 30 days prior to the expiration of the license.

Medication
Upon a review of Individual Health Care Plans (IHCP) at the program, one child's record did not contain a complete IHCP. The IHCP form must include the names of the educators trained in the child's medical condition and the name of who trained those educators.
The program did not have a Staff Schedule posted in the program. During the visit, the Director stated they updated this in January and haven't had the opportunity to print it out and post.
Upon a review of the BRC Navigator, the following was identified: -One educator was due for renewal 10/21/21. -One educator was due for renewal 1/8/22. -One educator is not listed in the BRC Navigator. -There is no Licensee identified for the program. All educators are listed as "employees."
Indoor Space
Upon a walk through of the facilities, the following was identified: -In the girl's bathroom there were two plungers and two toilet bowl cleaning brushes accessible to children.
