Two children's Individual Health Care Plans did not include the names of the staff trained on the child's chronic medical condition and will administer treatment while the child is attending the program.

The staff meeting log was reviewed. Meetings were not conducted for a minimum of 2 hours in January, February, March, April, and July 2024. There was no documentation showing a staff meeting was conducted in May 2024

-Attendance records were not accurate in the Preschool and Pre-K classrooms on 7/28/2022. In the Preschool classroom, there were 14 children present, but 16 were marked as present on the attendance list. In the Pre-K classroom, there were 10 children present, but 8 were marked as present on the attendance list. -When the licensor arrived, the Program Director was supervising 14 preschoolers in Room A. In addition, the classroom attendance showed that earlier in the morning there had been 16 children present. The second educator assigned to this group had an emergency injury at 10:45am that resulted in them leaving the program. The licensor learned that no second person had been assigned to that group from 10:45-1:30pm. The Licensee arrived to the program at 1:30pm and was assigned to work with the Program Director in the Preschool classroom. -Please submit a current staff plan that includes how groups will be in ratio, including during staff breaks.
This program does not have a Program Director working 100% non-teaching administrative hours. The Program Director is assigned to the Pre-School classroom full time. The Licensee covers breaks and staff absences. Please submit a current administrative plan.

During the visit, four children's files were reviewed. One file did not include a current Transportation Plan. One file did not include a current Developmental History Form.
During the visit, an educator in the toddler/preschool classroom walk one child out of the classroom, through a gate, and down the hallway to the bathroom for a diaper change, leaving the other educator alone with 7 children. Staff-to-child ratios must be maintained at all times.
During the visit, a hot/cold water dispenser was observed in the Infant classroom. The educators in the room stated the dispenser was used to fill a pan with hot water, and the infant's bottles were placed in the hot water to warm the formula. The safest solution is to serve bottles either cold or at room temperature. However, if parents ask to have an infant’s bottle warmed before feeding, the bottle may be held under warm, running water or placed in a container of warm tap water.
In the PreK Classroom, a box of vinyl gloves and pouches of diaper wipes were stored on shelves accessible to the children. In the Toddler/Preschool bathroom, a pouch of diaper wipes was accessible to the children
