Upon arrival, licensor observed a non-approved caregiver in the program. During the visit, the educator noted that the non-approved caregiver had been in the program to learn how to care for the children. The educator also noted that the individual had been assisting with the care and supervision of the children.

Upon arrival, the licensor was greeted by an individual who had not been approved by EEC to be present on the premises during child care hours. During the conversation, the educator stated that this individual had been in the child care program to learn how to care for the children. The educator also noted that the individual had been assisting with the care and supervision of the children.

The children's records were reviewed for the seven enrolled children. Two children required lead screening information on their file. Two children required annual enrollment update. One child was required the completion of the permission section (off premises) in their enrollment package.
Napping Materials
Upon arrival at the program, the Licensor observed a (REDACTED) child napping in a playpen that contained an additional mattress. Cribs, playpens, and bassinets must not contain any soft or padded materials that were not provided by the manufacturer.
One child who requires an Individual Health Care Plan did not have a completed plan on file, as required by EEC regulations.

Records were reviewed for the eight enrolled children. Three children require updated annual forms, one child is missing lead screening information, and two children have incomplete or missing information in the ‘Permissions’ section of their enrollment packets.
One child with documented food allergy or life-saving medication was not posted as required.

During the visit, licensor observed a six-month-old child napping in a bouncer chair. Two other infants under 12 months old were observed napping in playpens with additional mattresses placed inside.

First Aid Kit (FCC)
The following hazards were observed and accessible to the children: KITCHEN: Drawers were not secured containing sharp material inside including but not limited to knifes, openers, two large barbecue skewers. BATHROOM: There were a variety of beauty products on top and inside of the vanity including but not limited to skincare cleanse, exfoliants, sunscreen, shampoo, conditioner, deodorant, hydrogen peroxide, mouth wash, hair cutting corded machine, hair dryer machine.

Records were reviewed for the 9 enrolled children. Five children needed an Annual Update.
The inspection of the First Aid Kit revealed that two children need updated Emergency Cards.
Playground Safety - Fallzones
In reviewing the attendance logs, provider had not documented attendance since 06/02/23.

The playground observed did not have the required the fall zone of a minimum of 6 feet in all directions from the perimeter of the equipment.
Provider had her REDACTED who is visiting from REDACTED and an unapproved assistant staffing the program while she was off the premises.
Provider was present with ten children without a licensed assistant.
The front door exit was not marked with an exit sign.
Provider is not alternating exits on a monthly basis while conducting evacuation drill, does not have a meeting place and is not utilizing the correct evacuation drill form.
The gas grill in the outdoor space was uncovered and a small pool had sitting water. The fireplace's sharp edges was covered with a blanket which is not appropriately barricaded to prevent injury. Several electrical outlets were uncovered and plug in air fresheners were accessible to the children.

In reviewing the attendance logs, the sign out attendance for yesterday 05/30/23 was not complete and for today no one was signed in.

There were knives and plastic bags accessible to children in the kitchen. There was a fireplace with sharp stone edges accessible to children in the living room.

There were outlets in the lining room, dining room, and hallway that were not covered accessible to children.
Educator have not been maintaining attendance (assistant and children)
The children did not respond to the fire alarm, older children were stopping to look for shoes, there was no evidence of a plan has been discussed with the assistant.
Shoots for the dog are not up to date.

Debris and construction materials are piled on the the licensed outdoor space. Plastic slide is over 18 inches and does not have appropriate fall zone.
