It was observed in the PREK classroom that one child's water bottle was not labeled with their name.
Current consent for administered medications
The following was observed and accessible to children: 1) In TODDLER 2, that baseboard molding outside the bathroom area was missing. 2) In PREK, the light table electrical cord was unsecured.
A review of nine medications indicated that three were expired.
A review of the staff meeting log indicated that the meeting duration times were not documented and there was no documentation of a meeting held in March and June 2024.

A review of the illness, injury and incident log indicated that there was an entry from March 2025 that was incomplete and did not have a written report to match.
During the visit, the children's records checklist was not accessible or made available to the EEC Licensor. The Program Director stated that the Assistant Director only had access and was not present.
A review of six staff records indicated that two did not have evidence of bi-monthly observations between March-August 2025.
It was observed in the OUTDOOR play areas that on both climbing structures, the protective covering on the side and top of the stairs were peeling off and exposing metal edges.
It was observed that a current staff schedule was not posted.
It was observed that the first aid kits in INFANT 1 and 2 did not contain a pair of scissors and the first aid kit in TODDLER 2 did not contain tweezers and a thermometer.

A review of three staff records indicated that one did not have evidence of a current physical examination. The Program Director stated that they were aware and have requested documentation.
It was observed that there were small holes in the walls where shelves were moved from in TODDLER 1 and TODDLER 2.
It was observed that the refrigerator thermometers in the INFANT kitchen area and TODDLER 1 were above the appropriate temperature for food storage.

It was observed that there was chipped paint in the TODDLER 2 and PRESCHOOL 1 classrooms. The program is in compliance with their lead inspection.
It was observed that the Preschool play climber had spots of rust throughout and the protecting covering at the top of the stairs was peeling off, exposing sharp edges.
A review of three Indvidual Health Care Plans indicated that one was not fully completed to include the description of chronic health care condition and medical treatment necessary. All three did not list staff designated and trained to administer medication.

The following was observed and accessible to children: TODDLER 1 - The Dramatic Play area shelf was unsecured and tippable and the baseboard in that area was loose. TODDLER 2 - The Block area shelf was unsecured and tippable. PRESCHOOL 1 - The bookshelf was unsecured and tippable and the baseboard in that area was missing, exposing chipped and flaky paint.

In two rooms, the diaper pad was torn.
The following were observed indoors: -peeling paint (also outdoors on the gates) -There were several areas that had exposed plaster. -One preschool room had a peeling piece of baseboard trim. -One room had a piece of equipment (tee pee) leaning against the wall. This item was tippable.

The following were observed: -tod 2: The diaper pad was torn. -Tod 1: the diaper pad was torn and the bar on the paper roll was missing a cap. The bar was easily able to be pushed out.
There was an exposed metal piece protruding from the playground. There was rust and some chipping paint on the large composite structure.
This monitoring visit inspection recorded no violations or advisories.
A review of staff meeting documentation since the program opened indicated that there was no documentation of any meetings between May and July 2019. The January 2020 meeting did not include the lenght of the meeting.
The Garbage can in the PS 1 room was not kept in a covered container.

A review of 3 children's records indicated that one did not have a current physical examination. The last examination on file was dated (REDACTED).
During the visit, one child in the PS 1 room was disruptive and aggressive to an adult in the room and other children. Although the room was in ratio, the educator did not intervene quickly or use adult or peer support to encourage appropriate behavior and prevent challenging behavior.
The following were observed: -PS 1 and Pre K: There was missing baseboard trim. -PS 1: The inside of the fridge and wall behind the trash can, was soiled. One shelf was tippable (screws were not secured to wall). -Toddler 2: The counter around the sink, the front of the cabinets and child safety latch on the cabinet door was soiled. One piece of baseboard trim was missing. -Infant: The fridge and wall in feeding area were soiled.
The Individual Health Care Plan for one child, was not signed by his parent.
There was chipped paint throughout the program. There were several rooms that also had chipped plaster that was accessible to children.
