At the time of the visit the following was noted in the PS 2 classroom: 1. A swiffer wet mop and container of Swiffer wet pads was found stored on the floor in the bathroom accessible to children. 2. The wooden dramatic play kitchen was missing the plastic sink insert.

When reviewing medication the following was noted: 1. 1 child's prescription for an [REDACTED] had expired 1/4/24. 2. 1 child's[REDACTED]prescription stated to discard on 9/7/24 and the [REDACTED] itself had expired 11/24.

The program was closed for extensive renovations and a 2nd floor addition from January 2024 through November 2024. At the time of this Change of Space visit the following was noted: Window guards were not on 2nd floor classroom windows.

The program was closed for extensive renovations and a 2nd floor addition from January 2024 through November 2024. At the time of this Change of Space visit the following was noted: 1. Wooden shelving units, dramatic play sets/furniture was not secured in all classrooms.

At the time of the visit the siding on the building was cracked with sharp edges posing a potential hazard.
At the time of the visit 1 child's IHCP was not signed/dated by the parent.
At the time of the visit 1 child's [REDACTED] prescription stated to discard after 3/30/2023.

When observing JK classroom 3 the Licensor noted the tables were washed with soap/water only prior to snack. Tables must be washed then sanitized prior to eating.

Hand sanitizer and Cerve hand lotion were found stored on a low shelf accessible to children.

6 staff files were reviewed and the following was noted; 1. 2 files were missing the verification of 1 verbal reference. 2. 1 file was missing an updated PQ registry form. 3. All files were missing evidence of observations and consultations every other month.
IHCP's were reviewed and the following was noted: 1. When the Licensor asked an Asst. teacher if there were any allergies, chronic medical conditions in the classroom the Asst. replied, "I'm not sure I'm just an assistant". All Educators must be trained and aware of any child in their classroom with an allergy/ medical condition. 2. The name of the Educators who were trained in each child's plan was missing from some plans. 3. 1 child's IHCP had expired 1/4/24. 3. 1 child with a chronic medical condition that requires an [REDACTED] did not have an IHCP in file.
The PS 2 classroom had the children wash their hands then allowed them to return to the circle rug prior to eating.
