6 children's files were reviewed and the following was noted: 2 children had expired face sheet enrollment forms.
Through a review of 6 children's files it was learned, Progress reports were not completed on infants every 3 months and toddlers every 6 months.
The Step 2 climbing structure did not have 9 inches of impact absorbing material underneath it, it was located on grass.
The outdoor playspace had a monkey jump inner tube bouncer; the manufacturer's directions state that is to be used for children 3 years old and older.
1 child's medication ([REDACTED]was not listed on the allergy/emergency medication posting.
Written emergency evacuation procedures and diagram of escape routes were not posted by the Toddler 1 main classroom door.

At the time of the visit there was not enough shelving units with toys so that the children can independently access the toys.
The large white shelving units with drawers pose a potential hazard to children's fingers.
This investigation inspection recorded no violations or advisories.

Infection Control/Hand Washing - R Apr 2025
At the time of the visit the following was noted: 1. Cords to speakers were hanging down not secured and accessible to children in all rooms. 2. A swiffer wet jet was leaning against the wall accessible to children in the 2's classroom. 3. A broom was leaning against the wall and not secured int he 2's classroom. 4. A 6-plug adapter was plugged into the wall with no outlet cover protectors in the infant room. 5. A large box of changing table paper was found leaning against the feeding table.
At the time of the visit the Licensor noted the following, posing potential hazards: 1. Items were piled high on top of a low plastic storage shed. 2. A plastic blow up sprinkler was deflated and laying in the corner of the yard. 3. A garden hose was draped over the fence accessible to children.
The diaper changing pad in the infant classroom was sprayed with Spirit II it was not washed first with soap and water as required.

Of the 6 children's files randomly reviewed the following was noted: 1. 1 child was missing evidence of a physical exam. 2. 1 child was missing evidence of date of admission, 1st Aid Emergency Medical Consent & Release, Transportation plan, Developmental History, Physical exam and immunizations.

Progress reports were not in 2 infants files that had been at the program longer than 3 months.
1 staff file was missing a staff information form, documentation of interview date, evidence of the verification of 2 verbal references, and the BRC Suitability letter (Staff was verified Suitable by Licensor on site in the BRC Navigator).

Bottle Warming
A sink labeled hand washing only was being used for food prep in the Infant classroom; a bottle was being warmed inside this sink.

Attendance in the Infant classroom was incorrect, 1 child that transitioned from the toddler room to the infant room for a morning nap, was not signed in.
Infant bottles were being warmed with water above 120 degrees that was stored in a Thermos and then poured into a cup that was located inside the sink in the Infant classroom.
When observing in the Infant classroom the Licensor noted one child's diaper was changed and the child's hands were not washed.
