Attendance was not accurate in the Toddler 2 classroom 9 children were present and only 8 children were signed in.

An Educator in the Infant 1 classroom took both hands off of a child twice to put on gloves, while the child was being diapered on an elevated surface.
An IPad and plastic toy bins were found on top of the diaper changing surface.
When observing in classrooms the following was noted: 1. Several shelving units/dramatic play areas were not secured in the classrooms.
When observing in classrooms the following was noted: 1. PS 1 tables were not washed first prior to sanitizing as required. 2. A child's hands were not washed after diapering in the Infant 1 classroom. 2. The diaper changing surface was not washed prior to disinfecting as required.
1 child's IHCP stated to administer [REDACTED] and none was on site.

All files were missing a staff information form. 2. 1 file was missing evidence of 1 verification of references and 1 file was missing evidence of 2 verification of references. 3. 2 files were missing evidence of a completed BRC. 4. 2 files were missing evidence of [REDACTED] 5. all files were missing evidence of a completed orientation as it was not signed/dated. 6. 1 file was missing evidence of Teacher qualifications. 7. All files were missing evidence of the EEC Essential trainings prior to working with children.

3 Children's files were reviewed, and the following was noted: 1. 1 file was missing an updated face sheet enrollment form, 1st aid emergency consent and release form, Transporation plan, and developmental history. 2. 1 file was missing a physical exam, lead screening and immunizations. 3. all files were missing an offsite consent permission form.
At the time of the visit. In the toddler 1 classroom, there was not a sufficient amount of toys accessible to meet the needs of the children in care. There were no books, puzzles/manipulatives, or table/chairs for dramatic play area.
Written emergency evacuation procedures were not posted by the 2nd means of egress in the K-Prep classroom (the door that leads into another classroom).
At the time of the visit 1 Educator in the Toddler 1 classroom wiped a child's nose and did not wash her hands.

Several IHCP's were not signed and dated by the child's parent.
When observing in the Pre-K classroom the following was noted: 1. The Pre-K classroom exceeded the maximum group size of 20 children, there was 22 children present. 2. 2 Educators left 1 Educator alone with 22 children while they cleaned and set up the adjacent classroom.
Written emergency evacuation procedures were not posted by each means of egress out of the following classrooms: Nursery Pre-K, Nursery PS, and Toddler 1.
Documentation of evacuation drills showed evacuation drills were not conducted every month as required.

Individual Health Care Plan (IHCP) - R Apr 2025
Equipment and supplies: free of entrapment hazards
Edges to the rest mats in the PS 1 classroom were beginning to rip/tear and the inner foam was exposed.
When observing the playgrounds, the following was noted: 1. There was not the required depth of impact absorbing material underneath the swings on the PS playground which then resulted in the swings not having the required use zone in front of and behind them. 2. A picnic table was in the use/fall zone of the PS swings.
At the time of the visit the following was noted: 1. The emergency medications for each child was missing from the allergy/emergency medication posting. 2. The Toddler 1 classroom had the posting posted on the outside of the cabinet for all to see, it must be posted privacy protected.
1 child in the Nursery Toddler classroom's IHCP stated to administer [REDACTED] as an emergency medication and there was none on site to administer.
When observing in classrooms the following was noted: 1. The Nursery Toddler classroom had pom poms and jingle bells in the sensory table, these items are considered a choking hazard. 2. A window blind in the PS 1 classroom was broken and in need of replacement.

2 staff files were reviewed, and the following was noted: 1. Documentation of interview date was missing from both files. 2. Verification of 2 verbal references was missing from both files. 3. Evidence of having completed the 12 EEC essentials trainings were missing from one file.
When observing in the Toddler 1 classroom and gross motor room next door (door was open between rooms) the Licensor noted a child crying for several minutes, the child was trying to climb onto the Educator's lap as she was sitting on the floor in the doorway, the Educator kept saying "you're ok and you're fine", she would take the child off of her and put him down on the floor next to her. The Educator did not try to comfort or engage this child in any activities. The other Educator sitting on the floor was also not talking to or engaging children in play. When the 3rd Educator finished project, she sat down in the Toddler 1 classroom next to the Educator that was sitting in the doorway, there were no children present in that room, she did not join or interact with the group that was in the gross motor room.

3 children's files were reviewed, 1 file was missing evidence of a 2-year-old lead screening.
Attendance was inaccurate in 3 classrooms: 1. The Infant classroom; 5 children were signed in and only 4 children were present one child was transitioning into the Toddler 1 classroom. 2. Toddler 1 attendance showed 6 children signed in, 7 children were present. 3. PS attendance showed 8 children were signed in, 9 children were present.
When observing in the toddler gross motor room the Licensor noted the following: 1. Boxes, toys/items were piled high against the walls. 2. A screen to a window was leaning against a wall. 3. An easel was closed and leaning against the wall. 4. A plastic Little Tykes slide that is higher than 18 inches did not have the required 6 foot of impact absorbing material in the 6-foot area surrounding it.
The diagram of escape routes was missing from the PS classroom back door that leads into another classroom.
