Attendance was not accurate in 2 classrooms. The Kindergarten grouping; 8 children were in attendance from the Sunita Williams school, and none were signed in. 2nd grade and up classroom; 8 children were in attendance from the Sunita Williams school, and none were signed in. Once the Licensor brought it to their attention they were signed in immediately.

Emergency medications readily available
1 Educator employed longer than 6 months, did not have evidence of current first aid training.
At the time of the visit the following was noted: 1. Countertops in the bathrooms were worn down, with wood starting to bubble, they were not impervious to water therefore cannot be cleaned and disinfected as required. 2. 2 square floor tiles in the hall were chipped posing a potential tripping hazard.
When observing in classrooms the following was noted: 1. Written emergency evacuation procedures were not posted by each means of egress. 2. The diagram of escape routes was not accurate in all classrooms; rooms were numbered incorrectly or not at all, and not all rooms were showing on all diagrams (bathrooms).
Fire evacuation drills were not practiced every month as required; 9/2025, 2/2026 and 4/2026 were missing.
1 child's IHCP stated to administer Zyrtec and none was on site.
This investigation inspection recorded no violations or advisories.

3 staff files were reviewed, and the following was noted: 1. 3 files were missing evidence of annual evaluations. 2. 1 file was missing evidence of updating the EEC PQ registry. 3. 3 files were missing evidence of the required professional development training. 4. 3 files were missing evidence of staff observations/consultations every other month.
Green plastic fencing with metal poles was not secured, the poles were sharp and leaning, posing a potential impalement hazard.

At the time of the visit both ECE and SA children were observed filling their water cups out of a bathroom sink used for handwashing after diapering/toileting.
Equipment is clean, safe, secured, and poses no hazard.

3 staff files were reviewed, and the following was noted: 1. 1 file was missing interview date, verification of 1 verbal reference, evidence of a physical exam and [REDACTED] evidence of program orientation. and staff observations/consultations every other month. 2. 1 file was missing evidence of Group Leader qualifications, evidence of the required number of professional development hours for 2022, and staff observations/consultations every other month. 3. 1 file was missing evidence of [RADACTED] evidence of the required number of professional development hours for 2022, evidence of program orientation, and a yearly evaluation.

At the time of the visit the following was noted: 1. A spray bottle of green fluid was found stored on a low shelf in 2 classroom accessible to children. 2. The spray bottle was not labeled with its contents. 3. Bottles of hand sanitizer were found stored on shelves accessible to children in each classroom.
1 child's IHCP stated to administer [REDACTED] as an emergency medication and there was none on site to administer.
This enhanced monitoring visit inspection recorded no violations or advisories.
At the time of the visit the bleach/water solution was kept on the shelf accessible to children.

When reviewing Background Record Checks in the old and new system it was learned 1 Educator was hired and began working with children without undergoing a Background Record Check.
Staff monitor environment for hazards-Hazardous items not accessible to children
At the time of the visit there was no evidence in the ECE or SA program that staff meetings were being conducted with all staff 2 hours per month as required.
At the time of the visit attendance was not accurate in the SA classrooms. Educators did not sign children in with their time of arrival when they arrived at their classroom/grouping. Children were signed into more than one group at a time as they were moving children to accommodate ratios and they were not signed out of one group/classroom, so this classroom showed 15 children on the attendance but only 13 children were present.
Several round tables in a SA classroom were not in a clean, safe condition for eating. The coating of Formica had worn off in several areas exposing holes/the wooden fiber underneath. These tables cannot be appropriately cleaned and sanitized.
SA children were being sent to the bathroom 2 at a time unsupervised. Children can use the bathroom independently but only 1 is allowed to go at a time if there is no adult supervision.
Hand sanitizer was found stored on a shelf accessible to children in the K-1 homeroom.
