Shelving units and the dramatic play area were found turned around so the children could not access them in Rm 5.
When observing in classrooms the following was noted: 1. Sleeping bags/bedding were touching one another in Rm 11. 2. Rest mats were torn with foam exposed in Rm 11. 3. Several holes were found in the wall near the toilets in Rm 6 & Rm 11. 4. Cords to the phone and light table that were longer than 6 inches, were accessible to children in Rm 6.
When observing the outdoor play areas the following was noted: 1.There was not 9 inches of impact absorbing material in the 6 foot use/fall zones surrounding the climbing equipment and swings. 2. Water hoses around trees and across the I/T side yard posed a potential tripping hazard. When not in use it must be rolled up.
Written emergency evacuation procedures were not posted by each means of egress from classrooms
This investigation inspection recorded no violations or advisories.

An Educator in the 8B classroom sprayed both soap/water and bleach/water on the tables at the same time, it must be a 2-step process clean with soap/water then sanitize with the bleach/water, then allowed to air dry.

3 SA staff files were reviewed, and the following was noted: 1. 1 file was missing a staff information form, date of hire, and evidence the Educator was Group Leader qualified (education and work experience form). 2. 1 file was missing an annual evaluation, evidence of [REDACTED], evidence the Educator is Group Leader qualified (their transcript did not say a date in which a degree was awarded) and there was no work experience on file. 3. 1 file was missing evidence of [REDACTED]
When observing the playgrounds the following was noted: 1. Children were allowed to climb on the wooden stick hut, there is not a 6-foot use/fall zone completely surrounding that structure. 2. There is not 9 inches of measurable wood chips/mulch in the playground surrounding climbing structures/swings/slides.
When observing in the 8B classroom the following was noted: 1. Educators were changing children's diapers on top of their rest mats which were already made up with sheets/blankets on them. 2. Children's hands were not washed after their diaper was changed. 3. Educator's hands were not washed after they changed a child's diaper.
Equipment is clean, safe, secured, and poses no hazard.
The handwashing after diapering sink in Room 5 was full of toys/items.

When observing the outdoor play space the following was noted: 1. Empty plastic water bottles, cups and ziplock bags were found in the back playground. 2. 2 wires were found sticking out of the building and into the ground in the front playground accessible to children (near sprinkler alarm). 3. The platform to the concrete stairs that leads into the building near the front playground gate was rusted with sharp edges and there was a large space children could access and climb under.
An Educator in Room 5 did not wash the child's hands after diapering.
At the time of the visit the following was noted: 1. The travel bag which contained medications was found in a basket on the floor accessible to children in Rm 7. 2. An Educators bag was found stored on the floor accessible to children in Rm 4. 3. A Keurig hot coffee maker was found in a SA classroom.
1 child's IHCP stated to administer [REDACTED] and there was none on site.

16 Staff records were reviewed, and the following was noted: 1. 1 SA Educator was missing application/resume, documentation of interview date, and verification of 1 verbal reference. 2. 1 SA Educator was missing documentation of interview date, verification of 2 verbal references, updated physical exam and updated PQ registration. 3. 1 SA Educator was missing evidence of EEC's 12 Essential trainings. 4. 1 SA Educator was missing an updated physical exam. 5. 1 SA Educator was missing updated physical exam, EEC PQ registry date and evidence of First aid and CPR training. 6. All SA Educators were missing evidence of observations and consultations every other month.

16 children's files were reviewed, and the following was noted: 1. 2 ECE children were missing evidence of an updated lead screening. 2. The SA children's transportation plan on how the child arrives to the program was not clear, it stated bus or parent. 3. There was no sign off for parents to state the child's physical and immunizations is on site at the public school.
At the time of the visit the following was noted: 1. Several shelving units and dramatic play kitchen sets that were not on wheels were not secured and potentially tippable. 2.. A cord to a speaker was not secured in RM 15. 3. A basket of sunscreen was found stored on a low shelf accessible to children in Room 10.
The evacuation procedures and diagram of escape routes was missing from the SA Youth Community Room B.
At the time of the visit not all emergency medications were listed on the posting.
At the time of the visit the following was noted: 1. An Educator in Room 3 did not wash her hands prior to putting on gloves to serve snack. 2. An Educator and children in 4th grade did not wash their hands prior to serving/eating snack
When observing in classrooms the following was noted: 1. In Rm 5 the diapering surface was not sprayed first with soap/water prior to disinfecting, the Educator did not allow the disinfectant to remain wet on the surface. 2. Tables were not washed or sanitized prior to the children eating snack in the SA 4th grade room.

An Educator in Room 5 took both hands off of the child while the child was being diapered on an elevated surface.
This capacity increase visit inspection recorded no violations or advisories.
When observing classrooms, the following was noted: 1. A child in Room 2 filled up a cup of water to drink for snack using the bathroom sink. 2. In Room 16 the sink used for handwashing after diapering was full of bibs, toys and trays. Facilities used for handwashing after diapering/toileting must be separate from facilities used for food preparation and food services.

At the change of space visit the following was noted: 1. A window guard needs to be added to the window in the hallway stairwell that leads to the second floor.

When observing the playground the Licensor noted the following: 1. Discarded face masks and gloves on the ground outside and inside the main playground. 2. A net swing lying on the ground in the riding area.
At the time of the visit the following was noted: 1. Disinfectant wipes, and bleach/water spray bottle was found stored in cabinets under the sink that are accessible to children in Room 4. 2. Bleach/water spray bottles were found stored on top of the counter accessible to children in Room 4. 3. Sunscreen was found stored in children's cubbies in Room 4 and Room 6.

Emergency medications [REDACTED] were not on site for 2 children with chronic medical conditions in the Community Room 2 School-Age classroom.
At the time of the visit one child's bottle of [REDACTED] had expired [REDACTED].
