An Educator in the infant classroom wiped a child's nose and did not wash her hands.
When observing in classrooms the Licensor noted children waiting for an extended period of time. 1. In multi-age 2 the children were waiting in chairs/tables for over 20 minutes while children showed a toy for show and tell. 2. Toddler 3 children were seated at the table waiting for the teacher to cut out/prepare project. 3. Toddler 2 9 children were seated at the tables and children were made to wait for the educator to give them a flower and glue. Only 1 glue bottle was used, and the Educator put the glue on the paper for the children. It was over 20 minutes by the time they got to the last child.
Educators in the Infant room and Toddler 2 classrooms took both hands off of the child when the child was being diapered on an elevated surface.
It could not be determined if staff meetings were held for a minimum of 2 hours per month due to the program not documenting the date and meeting time.

When reviewing medication the following was noted: 1. 1 child's epi-pen expired 11/2025 (the prescription was still valid).
When observing in classrooms the following was noted:1. The diaper changing surface in the Infant and Toddler 2 classroom was not disinfected after washing it with the cleaning solution as required. 2. The Educator in the Toddler 2 classroom did not wash her hands after changing a child's diaper. 3. An Educator in the Toddler 2 classroom did not take off her gloves and wash her hands after cleaning the diaper changing surface, she picked up the next child and started to change them.
6 Staff files were reviewed and the following was noted. 1. All staff did not have evidence of the required number of professional development hours on file. 2. 4 Educators did not have evidence of [REDACTED] on file. 3. 3 Educators did not have evidence of a current physical exam on file. 4. 2 staff did not have evidence of current first aid on file.

6 children's files were reviewed and the following was noted: 1. 1 file was not on site the Educator gave it to the parent to resign/date forms. 2. 1 child's face sheet enrollment form was not signed/dated by the parent. 3. 3 files were missing evidence of a lead screening. 4. 2 files were missing evidence of immunizations.
When observing in classrooms the following was noted: 1. The front and side of the formica countertop to the sink in the Multi-Age 2 classroom was peeling off causing the wood underneath to expand and large holes. 2. The board under the sink in the Toddler 1 classroom was detached and hanging down onto the floor. 3. The plastic sink insert was missing from the dramatic play sink in the Toddler 2 classroom. 4. The staff bathroom floor and sink were dirty and in need or cleaning.
2 cement pilings that support the fence posts were sticking out of the ground posing a potential tripping hazard on the Toddler playground.
The allergy/emergency medication posting was missing the name of the actual medication that was prescribed to the child [REDACTED]
Written emergency evacuation procedures and diagram or escape routes were missing from several classroom exits.
Cleaning and disinfecting bottles were found stored under an unlocked changing table accessible to children in the Toddler 2 classroom.

The thermometer in the Infant refrigerator was broken so it could not be determined if the fridge was between 32-40 degrees for safe food storage.

At the time of the visit the Licensor observed a child go from the Multi-Age 3 classroom with 2 Educators to the Front desk with 1 Educator to the Multi-Age 1 classroom (which is his scheduled classroom) to an empty Infant classroom with 1 Educator. The child was not assigned to a consistent classroom with consistent staff.

When observing in classrooms the following was noted: 1. A Table in the Multi-age 2 classroom was not washed and sanitized prior to a child eating snack at it. 2. An Educator in the Toddler 3 classroom did not wash her hands immediately after changing a child's diaper, she cleaned up books/toys then washed. 3. In the Toddler 1 classroom the diaper changing surface was not washed and disinfected after each child.
4 Children's files were reviewed, and the following was noted: 1. 2 files enrollment packets were not signed/dated on each page. 4 files were missing evidence of progress reports, 1 developmental history was not signed/dated, and 1 developmental history had expired 3/24, 1 file was missing evidence of an updated physical exam, 1 file did not have a physical exam, 2 files were missing evidence of updated lead screenings.

3 staff files were reviewed and the following was noted: 1. 1 file was missing a staff information form, documentation of interview date, annual evaluation, physical exam, MMR's and First Aid training. 2. 1 file was missing annual evaluation, evidence of updating int he PQ registry, evidence of updated first aid training.
At the time of the visit the following was noted: 1. Emergency medications such as [REDACTED] were not listed on the posting. 2. The Toddler 1 classroom had post it notes with confidential allergy information posted on top of the allergy list for all to see.
At the time of the visit a staff member reported that a child had run away from an Educator out the playground gate and down the path to the roadway. The program did not have any documentation in the child's file of this incident.
At the time of the visit the Toddler 3 classroom was disposing diapers in a trash can that did not have a lid.
The written emergency evacuation plan was missing from the Multi-age 2 classrooms back door that leads into another classroom.

Bottle feeding
An Educator in the Infant 1 classroom was not holding an infant while feeding a bottle, she had the infant lying on a boppy pillow while she held the bottle to feed.
An Educator in the Toddler 3 classroom left the room leaving one Educator alone with 9 children.
6 staff files were reviewed, and the following was noted: 1. All were missing evidence of a yearly evaluation. 2. 3 were missing evidence of a current physical exam. 3. 1 was missing evidence of [REDACTED] 4. 1 was missing evidence of the 12 EEC Essential trainings. 5. All were missing evidence of professional development hours. 6. All were missing evidence of bimonthly observations and consultations.
When reviewing IHCP the following was noted: 1. 2 Educators in the Toddler 3 classroom stated they were not trained in a child's IHCP and medications. When asked one Educator stated there were no [REDACTED] but there was medication for one child in the emergency backpack, the Licensor checked and found an [REDACTED] 2, The Toddler 3 child's IHCP did not state which Educators were trained in that child's specific IHCP. 3. 1 IHCP in the Multi-age 2 classroom had expired 5/16/2023.

6 Children's files were reviewed and the following was noted: 1. 1 file was missing evidence of a First Aid medical consent & release form, transportation plan, developmental history, physical exam and immunizations. 2. 1 file was missing evidence of an updated face sheet. 3. 1 file was missing evidence of an updated First aid emergency medical consent & release, transportation plan and developmental history. 4. 4 files were missing evidence of progress reports.
When observing in the Toddler 1 classroom the Licensor noted the following: 1. There was not a sufficient amount of toys and equipment on shelves accessible to children. 2. There was no dramatic play food, clothing, accessories, table or chairs to foster dramatic play. 3. The Educator put the doll house on the table for the children to play with however, there was only a couple of pieces of wooden furniture inside, they did not provide the children with people or props to foster play.
When observing in classrooms the following was noted: 1. 2 Educator purses were found stored accessible to children in the Toddler 3 classroom. 2. A toilet cleaning brush was found stored accessible to children in the Toddler 3 bathroom. 3. Dramatic play plastic sink inserts were found missing from several sinks in classrooms. 4. Several ceiling tiles were found to be stained and in need of replacement.
Written emergency evacuation procedures were not posted by each exit out of classrooms.
When reviewing the fire drill log the licensor noted drills were not conducted in October 2023 and February 2024.
Diaper cream and lotion was found stored under a diaper changing table in the Toddler 1 classroom. The cabinet was not locked so it was accessible to children.

At the time of the visit the following was noted: 1. Children were not washing their hands after using the bathroom in the Multi-Age 1/PS A classroom. 2. A child's hands were not washed after a diaper change in the Infant A classroom.

At the time of the visit the following was noted: 1. An adult size plastic chair that was cracked with sharp edges in the Multi-Age 1/PSA classroom. 2. An Educator purse was found stored accessible to children in the Multi-Age PS/A classroom and Toddler 2 C classroom.
The written emergency evacuation procedures and diagrams of escape routes were not posted by all means of egress in classrooms.
At the time of the visit the following was noted: 1. Tables in the Multi-Age/PS A classroom were not washed with the sanitizer after snack. 2. The diaper changing table was not disinfectanted after it was cleaned with the blue cleaning spray.

When observing in classrooms the following was noted: 1. Diaper cremes were found in children's bathrooms (Two B & C) accessible to children. 2. A spray bottle of disinfectant was found stored on top of a mat accessible to children in the Two C bathroom. 3. Several spray bottles of cleaners/disinfectants were found stored in a low cabinet that did not lock, they were accessible to children in Two C.

2 Staff files were reviewed and the following was noted: 1. 1 file was missing a staff information form, evidence of a physical exam within the last year and immunity to MMR, evidence of having completed all 12 Essential EEC required trainings and a program orientation. 2. Both files were missing evidence of staff observations and consultations every other month as required.
When observing in classrooms the following was noted: 1. In the Toddler A classroom, the paper towels were lying on the diaper changing table and not in the paper towel holder as required and all paper towel holders were empty in this classroom. 2. The soap dispenser next to the children's sink in Toddler A did not have soap in it. 3. The toilet paper holder was broken/missing its cover in the bathroom in the PS middle room that the TK children were using.
An Educator in the TK grouping re-filled the water pitcher in a sink that is used for handwashing after toileting.
When observing in the Infant A and Toddler A classroom the Licensor noted the following: 1. Educator's and children's hands were not washed after each diaper change. 2. The diaper changing surface was only cleaned with the glass cleaner it was not disinfected as required after each diaper change.

At the time of the visit the Licensor noted the following: 1. A child in the [REDACTED]classroom was seated at the table their feet did not touch the floor and the table was above armpit height; this is not considered developmentally appropriate. The same child could not reach the water in the sink to wash their hands; a stool must be provided.
On the day of the visit the daily schedule said Toddler 1, the roster submitted to EEC said Toddler A and the clipboard attendance sheet (CSR) said Toddler B. This is not considered accurate record keeping.
When observing in the “[REDACTED]” classroom the Licensor noted the following: 1. A child blew their nose and did not wash their hands after. 2. Both Educators did not wash their hands prior to putting on gloves to serve lunch. 3. Tables were not cleaned and sanitized prior to the children eating lunch.

When observing in classrooms the following was noted: 1. Boxes and bags of clutter were found piled high on top of the windowsill in the "Pre-K 2" classroom posing a potential hazard. 2. Dustpans and brooms were found leaning up against walls and not secured in the "Toddler A" and "DPS B" classrooms. 3. A toilet bowl cleaning brush was found stored in the "DPS B" classroom accessible to children. The program was previously cited for this on 12/16/19. In a correction plan dated 12/23/19 the program stated: "1.All teachers where instructed to use the broom latch to secure it to the wall found in the classrooms. 2 .toilet bowl plungers and cleaners were removed from restroom and teachers were instructed not to store them in the bathrooms. Classroom bathrooms will be check regularly to make sure that this does not happen again."
At the time of the visit, the attendance record in the "DPS A" classroom was not accurate; 8 children were present and only 5 children were signed in on the attendance sheet.
At the time of the visit, children were not assigned to a consistent group with consistent staffing. The Licensor noted 7 out of the 11 classrooms had children in them that were not the child's assigned classroom.

At the time of the visit, the enrollment freeze letter issued to the program on 12/17/19 was not posted with the program's current license as required. When the Licensor entered the building the enrollment freeze letter was lying on the shelf of the front desk; the [REDACTED] stated she was told to put it there by the [REDACTED]. The Enrollment freeze letter states in bold: "This letter must be posted conspicuously with the current license". The [REDACTED] immediately posted the enrollment freeze letter next to the current license.
At the time of the visit the staff schedule posted was not current. One new Educator was hired and two Educators had quit.

At the time of the visit, the Licensor noted the "Toddler 1" classroom was not following the program's name to face attendance policy when transitioning from the classroom to the gym. The Educators did not complete a name to face roll call prior to exiting the classroom with the group of toddlers.
Of the [REDACTED] Children's files randomly reviewed the following was noted: 1. 1 child's file was missing evidence of [REDACTED], [REDACTED ]file was missing evidence of a progress report every 6 months and [REDACTED], and [REDACTED] file was missing evidence of an updated [REDACTED] and progress reports every 6 months.
At the time of the visit, [REDACTED] was observed sleeping in a crib that was not labeled for his use.
At the time of the visit, it was determined the program was not being soundly administered. During the visit several non-compliance's were identified including but not limited to: *An unqualified staff was responsible for a group of children. -*There was no evidence of a completed Background Record Check. *Exceeded the licensed Infant capacity. *Expired emergency medications[REDACTED]

At the time of the visit the program did not have evidence of a completed Background Record Check (BRC) on file for 2 Educators; 1 Educator was never entered into the BRC system at all and the other Educator never completed the BRC process, she did not get fingerprinted.

When observing the playground the Licensor noted the poured rubber surfacing on the PS playground was peeling in between the slide and tunnel posing a potential tripping hazard. The program was previously cited for this. A correction plan dated 7/1/19 that was submitted to EEC stated: "Work orders have been submitted and we are in the process of receiving quotes for the work to be completed. Work will be completed by August 27, 2019."

At the time of the visit the written emergency evacuation procedures and diagram were not posted by internal doors leading from Infant 2 to Infant 3 and vice versa.

At the time of the visit the following was noted: 1. 2 Educators were assigned to 2 different classrooms as the Teacher qualified Educators; however; the program had no evidence that the 2 Educators were EEC Teacher qualified. The Licensor checked the Teacher Qualification database and found 1 Educator to be qualified and[REDACTED] that was not qualified who received a letter in 2018 stating [REDACTED] was not qualified. 2. 1 Educator who was covering breaks in the Infant and Toddler classrooms for Teacher qualified Educator's did not have evidence on file that she was Infant/Toddler Teacher qualified as required.

At the time of the visit the attendance was not accurate in the Infant 1 classroom.
At the time of the visit, the program was exceeding its current licensed infant capacity of 14 infants. Documentation submitted to EEC shows the program was providing care for up to 22 infants at one time in the Infant 1, Infant 2 and Infant 3 classrooms.
When observing in classrooms the Licensor noted garbage/food refuse was being thrown into trash cans that did not have a cover as required.
When observing in the "Infant 2' classroom the Licensor noted [REDACTED] lying on a boppy pillow on the floor drinking a bottle.
When observing in classrooms the following was noted: 1. Dust pans and brooms were found leaning up against walls and not secured in several classrooms. 2. Toilet bowl plungers and toilet bowl cleaning brushes were found stored in several bathrooms accessible to children. 3. A plastic sink insert was missing from the dramatic play sink in the "Toddler 2" classroom.
At the time of the visit, the allergy/emergency information/medication posting was not current and accurate, it was dated August 2019 [REDACTED] which also required medication was missing from the posting.
At the time of the visit, [REDACTED] was assigned to and working alone with a group of children in the "DPS 1" classroom. Assistant Teachers must work under the direct supervision of a Teacher qualified Educator at all times.
At the time of the visit, the program was not maintaining Teacher:Child ratios. There was not a Teacher qualified Educator assigned to the "DPS 1" classroom.

When observing in the "Toddler 2" classroom the Licensor observed 2 spray bottles of cleaners stored in an open cabinet accessible to children. When it was brought to the Educator's attention she closed the door to the cabinet however, the cabinet safety lock was broken. The Educator was told to move the toxic substances up to a high shelf, out of reach of children.
When observing in the "Toddler 2" classroom the Licensor noted the children did not wash their hands prior to eating lunch.
At the time of the visit an Educator took both hands off of a child that she was diapering on an elevated surface; she took both hands off to reach for gloves and to put a bottle of cleaner underneath the diaper changing table.
Of the 5 [REDACTED] reviewed the following was noted: 4 [REDACTED] were expired and 1 [REDACTED] was missing the parent's signature/date.
At the time of the visit [REDACTED] stated to [REDACTED]; however, there was none on site to administer.
