At the time of the visit the program did not have one Educator on site with evidence of CPR. The Regional Director came after some children had arrived and she was able to show evidence of current First Aid and CPR.
3 children's files were reviewed and the following was noted: 1. All files were missing the date of admission.
3 staff files were reviewed and the following was noted: 1. 1 file was missing evidence of education and work experience, [REDACTED] First Aid and CPR 2. 2 files were missing evidence of education, First Aid and CPR.
1 child's transportation plan was not accurate it stated they depart from the program by the program van/bus which is not accurate.
Written emergency evacuation procedures were not posted at both exits; they should be posted with the diagram.
1 child's IHCP stated to administer [REDACTED]and there was none on site for this child.
When reviewing IHCP's the following was noted: 1. 1 child's IHCP had expired it was signed/dated 4/2023, it is only valid for one year from that date. 2. 1 was missing the name of the Educators who were trained in the IHCP.
Tables were not washed with soap/water and then sanitized prior to the children eating lunch at them. Tables also contained toys, games, markers etc.. and the children were observed eating around these items.

At the time of the visit the program was out of ratio for approximately 22 minutes as the children were arriving; the Site Coordinator was alone with 14-26 children from 12:30-12:52PM.
An Educator did not wash their hands prior to helping the children peel clementines for lunch.
Written emergency evacuation procedures were not posted at either exit.

3 Educator files were reviewed and the following was noted: 1. 1 file was missing application/resume, documentation of interview date, verification of 2 verbal references, and evidence of a physical exam. 2. 1 file was missing staff information form, application/resume, verification of 2 verbal references, date of hire, evidence of a BRC, program orientation form, and PQ registration. 3. 1 file was missing documentation of interview date, verification of 2 verbal references, evidence of a BRC, evidence of Group Leader qualifications, PQ registration and evidence of First Aid within 6-months of employment and evidence of completing the 12 EEC Essential trainings.

When reviewing emergency medication, the following was noted: 1. 2 children with a diagnosed medical condition [REDACTED] did not have emergency medication at the program. 2. 1 child's IHCP stated to administer an [REDACTED]and there was no [REDACTED] on site at the program. 3. 1 child's IHCP stated to administer [REDACTED] there was none on site to administer, the medication on site was from a foreign country it could not be determined if it was the equivalent to [REDACTED]
When reviewing IHCP's the following was noted: 1. 2 children with a chronic medical condition (asthma) did not have an IHCP on file. 2. 1 child's IHCP was not signed/dated by the parent. 3. IHCP's that were on file were missing required EEC information; what Educators received training in that specific child's plan.

Date of Hire was missing from all 3 files. 4. Evidence of completion of the EEC Essentials trainings was missing from 1 file.

Children's files were reviewed, and the following was noted: 1. 1 file was missing date of admission and an IHCP.
The van drivers log did not have evidence of a complete vehicle inspection to ensure all children have exited after each trip, nor did it have evidence that a second person is inspecting the vehicle to ensure no child is left behind.
Fire drills were not being conducted monthly with all groups of children as required.
1 child's medication administration form was not signed/dated by the parent.
When reviewing IHCP and medications the following was noted: 1. 1 child with a chronic medical condition that requires an epi-pen did not have an IHCP on file. 2. 1 child's IHCP was not signed/dated by the parent.

When reviewing children's records, the following was noted: 1. Date of admission was missing from the children's files.
2 Staff files were reviewed and 1 file did not have evidence of a completed BRC.
At the time of the visit the following was noted: 1. The attendance sheet was not accurate several times throughout the day, children had arrived and were not signed in immediately. 2. In the 2 Group Leader staff files reviewed, there was no evidence on file that these staff were GL qualified.
At the time of the visit a child was served a bag of pre-sliced apples that had expired. The child noticed the expiration date, the Educator did not check it prior to serving.
At the time of the visit the allergy emergency medication posting was not accurate. It had a child listed with an incorrect last name.
At the time of the visit the following was noted: 1. Staff did not wash their hands prior to putting on gloves to serve snacks/lunch to children. 2. Tables were not sanitized after washing them with a soap/water solution prior to eating lunch.
At the time of the visit the soap/water solution was stored in a bottle labeled as a disinfectant.
