During random review of 7 staff files the following was found: 2 files did not have full evidence of [REDACTED] and 1 did not have a current physical.
There were no medication consent forms on file for 3 different children's medications maintained on site.
During last renewal the programs licensed capacity was lowered. The program overenrolled children and exceeded their current capacity for the 2024/2025 school year without submitting an application for a license change prior to the children attending.

During health and safety inspection the following was found: broken molding at base of half wall in preschool bathroom.
Inspection of outdoor space found a rusted bike and a large cylinder within the use zone of the preschool swings.
Not all playground equipment/structures had required amount of impact absorbing material.
Program was found storing the [REDACTED] that come in one prescription box separately leaving [REDACTED]with no prescription information.
When asked an educator stated they only used a 1 step process using Force of Nature to clean and disinfect.
Program had 2 IHCP's for one child that stated different instructions on each.
Program did not have consent for medication administration on file for all medications.
