One child's IHCP stated only an [REDACTED] for emergency use and then had a separate medication order for [REDACTED] that only stated for allergic reactions. It was not clear when the [REDACTED] should be used. All required emergency medications must be noted and clearly explained on an IHCP.

Two children each had 2 different IHCPs on site. Each had one plan that was current and the other was not. Each had one plan that stated [REDACTED] as part of the action plan and the other plan did not. The FARE allergy forms provided by the doctor were expired as of 4/2024.
