Licensor randomly reviewed 4 of the children's records. Of these, 2 were complete and up to date, 1 had out of date signatures in the enrollment packet, and the 4th had no physical (only an immunization record). Please review all of the children's records and ensure they are all complete and up to date.

Licensor randomly reviewed 4 of the children’s records and none of them had an up to date progress report. There were 3 reports that were out of date and the 4th record had no progress report and the child has been enrolled for nearly 1 year.
All of the required emergency numbers/information were posted in the program, except for the program's address and the closest medical facility, which need to be added.
Licensor inspected all of the licensed, indoor space, as well as the outdoor playspace (back yard). The following hazards were observed: ** INFANT/TODDLER ROOM **- cords hanging down need to be secured to the wall so a child can’t pull them down ** ACTIVITY ROOM **- no hazards ** KITCHEN **- drawer on the left side of the island had small chokable items inside and the child safety latch was not engaged, drawer to the right of the dishwasher has a lighter and small drill bits inside, child safety latch was not engaged ** OFFICE **- no hazards ** BATHROOM **- no hazards ** OUTSIDE **- no hazards
For hand washing, the bathroom is equipped with liquid soap and the children use a common towel for drying. Regulations states that a common towel cannot be used. The children must either use individual towels, labeled for each child, or paper towels.

Licensor randomly reviewed 5 of the children's records. Of these, 2 were complete and up to date, 1 had no signature on Pg. 5 of the enrollment packet under Basic Transportation and no evidence of lead testing, 1 only had immunizations, no physical or evidence of lead testing, and the last record had lead testing that was out of date. Please review all of the children's records and ensure they are all complete and up to date.

Licensor reviewed 7 of the children's records. Of these, 3 had a complete and up to date progress report, but 4 either had no report or the progress report was out of date. Licensor reviewed the timeframe of when reports need to be completed for each age group.
No immediate health risks were observed during the monitoring visit. All of the required items were observed in the first aid kit. However, the children’s emergency contact forms were out of date. Tested the combination smoke and carbon monoxide detector in the utility room and it worked appropriately.
Educator completed 10 hours of professional development in 2020, and 5 hours in 2019 plus the EEC essentials. However, Educator was unable to locate her training certificated from October of 2018 to September of 2019. None in 2018

There was also 1 record that was missing the signature page )pg.6) of the enrollment packet. Please review all of the children's records and ensure that they are all complete and up to date.
Educator does not have complete or up to date records for any of the assistants that work in the program. These records must include the assistants' professional development certificates that have been completed.
Educator's license was not posted during the renewal visit.
There is a child in the program who has an allergy and is prescribed an EpiPen. Educator does not have an IHCP on file for this child.
