Educators not providing adequate supervision of children in the hallway as they were leaving their classrooms and going to the bathroom. Multiple children were observed going in the girls bathroom and no educator stood at the door to ensure proper supervision.

Educators are not providing appropriate supervision of children. During visit the following was observed: 1. An educator left children unattended in classroom while they went to the kitchen to make popcorn for snack. 2. A child was observed leaving a classroom to go outside unattended 3. A child was observed sitting alone in the hallway 4. A child was seen coming out of the kitchen and no educator was present 4. Two children came walking down the hall unaccompanied when they were done with Chinese class. Observations clearly showed educators and children freely walk in and out of rooms without communicating which is a potentially hazardous situation. The program is located in a church, in which people from the public have access. Program needs to submit detail plan on how they will ensure all educators provide appropriate supervision.

Licensors found a [REDACTED] sitting in one of the classrooms at 3:05 pm and a student arrived at 3:07 pm. [REDACTED] stated [REDACTED] is only at program on Tuesdays and [REDACTED] supposed to be there at 3 when children arrive. [REDACTED] stated [REDACTED] do not arrive until 3:30. When [REDACTED] arrived it was learned the [REDACTED] is from the [REDACTED] and is [REDACTED] and that [REDACTED] was not scheduled to be there that day. [REDACTED] unaware of EEC's [REDACTED] regulations that state [REDACTED] cannot be left alone with children, be counted in ratio or have a completed background record check. Submit plan to ensure the program is in compliance with EEC's [REDACTED] regulations.

There were no staff present when middle school children arrived. Attendance was not taken until 3:53pm when the Administrators were reminded to. Attendance needs to reflect actual arrival and departure times of all children.

Random review of 6 staff files found the file of a new hire without evidence of group leader qualifications and no evidence of Strong Start Essentials 2.0 training.
A new educator was alone with a group of children and there was no evidence on file they were at least group leader qualified.
Attendance roster on wall was not completed after children arrived at the program.
When Licensor arrived, children were observed in a classroom with no educator present and other children were observed leaving other rooms to go to the bathroom and no educators present to properly supervise. Program has been cited repeatedly for lack of supervision.
Not all open snack items were stored in an air tight container.
When Licensor arrived there were 5 middle school children alone with no staff on the premises. Program has been cited for this prior. Administration states the children are not supposed to be dropped off early. Please submit a plan arranged with parents to ensure this does not happen in the future.
Staff had to be reminded to sign in the middle school children that had arrived early.
Evacuation drills have not been conducted since January 2022.

Review of 3 children's files found the following: 1 file did not have a signed/dated 1st aid/emergency consent and 1 file did not have a transportation plan.
Review of CPR training found that no staff had current CPR training at time of visit.
The program is not being soundly administered. Administration has not ensured an adequate level of supervision of children in their care and has not ensured the program is set up and ready for children upon arrival.
A staff person alone with a group of children did not have group leader qualifications on file.
Administration exercised poor judgment by not having staff present when children arrived at the program. Middle school children arrived at the program at 3:05pm and [REDACTED] did not arrive to the program until approximately 3:35pm. A [REDACTED]arrived, but was scheduled to work with a [REDACTED] and their mother on the first floor and the children were on the second floor.
[REDACTED] person was observed using their hands to give out crackers and cereal. [REDACTED] was not observed washing their hands prior to serving snack and they did not wear gloves.
Box of cereal used at snack was opened and not sealed properly.
Not all children's hands were washed prior to snack. Tables were not cleaned and disinfected using a two-step process.
