When Licensor arrived there were 64 children in care with 3 educators. One of the educators was observed walking back from their car while they were considered in ratio. It was also learned 2 educators called out and 2 assistants would arrive shortly after 3pm. Please submit plan on how you will ensure you maintain ratios at all times.
No posting with documentation of children who have chronic medical conditions and/or emergency medication.
A child's IHCP expired 3/2026. Another child has an [REDACTED] and the program did not have an IHCP on file for this child.
The following medications were found expired: a child's epi expired 12/2025, a child's Benadryl expired 6/2025 and another child's epi expired 3/2026.
Although the Director reports having frequent staff meetings when program ends there is no documentation of the meetings.

Random review of 4 staff files found the following: 2 files had no first aid card and 1 had a card that did not state pediatric/child, 1 did not have evidence of a current physical, 2 did not have adequate evidence of group leader qualifications and 2 did not have evidence of Essentials training.
No evidence of written observations of all staff at least every other month.

When children entered the program they were observed sitting down at the tables and eating their snack without proper handwashing.
