During the visit it was identified that one child's emergency medication expired on 11/30/2024.

The following was observed during the visit. Two curved shelves in the Toddler 4 classroom and one curved shelf in the Preschool 1 classroom were not secured to prevent them from tipping.

In the Toddler bathroom, vinyl gloves, a plunger and a large dustpan were stored in an area accessible to the children. In the Preschool bathroom, plastic bags, vinyl gloves and a plastic pouch of diaper wipes were located in an area accessible to the children.
During the visit, peeling paint was observed on the exterior of the building.

During the visit, four staff files were reviewed. One file did not include a Staff Information Form. Two files did not include verification of two references. Two files did not include a current physical. One file did not include a current PQ Registry.
Furniture in the Toddler 4 Classroom was not secured to prevent it from tipping over.
The following hazards were observed on the playground and accessible to the children: In the preschool area, tree roots were protruding upward, causing a tripping hazard. The sides to the sandbox are broken. The wood trim is hanging off the building in the enrichment area.
During the monitoring visit, two staff files were reviewed. Two files did not include documentation of a current physical. One file did not include evidence of immunity to Measles, Mumps and Rubella. One file did not include evidence of Program Orientation. One file did not include documentation of the EEC Essentials 12 trainings. Observations of educators while working with children were not conducted at least every two months by a staff with lead teacher, site coordinator or higher qualifications. The Staff Record Checklist provided during the visit was not accurate.
A practice evacuation drill was not conducted in February 2022. Evacuation drills must be conducted monthly.

Food Storage, Sanitation & Safety
In the Toddler 4 classroom, a bottle of Goo Gone was on the counter accessible to the children. It was removed during the visit.
During the visit, the following was noted: there are stained ceiling tiles in the Preschool 1, Preschool 2 and Preschool 3 classrooms. In the children's bathrooms, brooms and dustpans were accessible to the children. In the Toddler 4 classroom, there is a hole in the wall by the bathroom. In the Toddler 3 classroom, the lock on the draw containing diaper creams is not in working order. In the Kindergarten Prep classroom, the white wall cover is loose and must be re secured.
Equipment is clean, safe, secured, and poses no hazard.
Evacuation procedures were not posted at the rear exit in the Kindergarten Prep classroom.

One educator did not have current first aid certification as required by EEC.
The following hazards were observed indoors and were accessible to children: In the Gross Motor Room, an educator's purse was stored on the window sill and accessible to the children. The purse was removed during the visit. The furniture was not secured in four classrooms. In the Preschool 1 classroom, there was a hole in the wall. The electrical power strip did not include safety covers in the receptacles that were not being used. There was a stained ceiling tile in the K Prep 2 classroom. In the Toddler2 classroom, the refrigerator must be cleaned and there is tape residue on the exit door.
The program's Fire Inspection expired on 4/2/2022. Please submit a current inspection to EEC.
The evacuation procedures were not posted next to the exit in the Gross Motor Room.

In the Infant 3 classroom, one electrical outlet was missing a safety cover to prevent the children from accessing the receptacle.
A child was observed sleeping with a (REDACTED). The program did not have medical documentation for this child regarding the use of this (REDACTED) while in care.
Five classroom first aid backpacks did not include a CPR mouth guard as required.
EEC licensing staff observed an educator in the Toddler 2 and Toddler 4 classrooms wash their hands in the food preparation sink after changing a child's diaper.
In the kitchen, there was one open package of cereal and an open container of flour not stored in covered containers.
