One educator's BRC was not current during the visit and had been working (expired May 2025).
A container of cheerios in a classroom and a container in the food storage area were not labeled with the expiration date of the food.
The following was observed to be accessible to children during the visit: In TODDLER, one ceiling tile near the door was stained and two short bookshelves in the middle of the room were not fully intact (top corners were peeling/exposed particle board); in DRAGONFLIES, the kitchen set with sink was not secure and tippable and two electrical cords were long and loose (the TV cord on the left side of the room and the extension/power strip on the left side of the room); and in FIREFLIES one electrical cord to the speaker was hanging long and loose. The space must be free from indoor/outdoor hazards.

One child's (REDACTED) was not available at the time of the visit. When BUTTERFLIES left the classroom to walk down the hall to use the bathrooms, the bag with emergency medications was left in the classroom. Emergency medications must be available at all times and should travel with the child.

The following was observed to be accessible to children: -The right side bathroom had a toilet brush sitting to the right of the toilet. -The left side bathroom had loose gloves in an unlocked plastic drawer and wipes in bags sitting on top of the drawers and in the drawers. The gloves are a choking hazard and the bags are a suffocation hazard.
The following was found to be accessible to children: -A crack is on the middle of the rock wall and has sharp edges that could scratch a child's skin.
Three child records were reviewed and found: -One record was missing the child's allergy plan.

Four child records were reviewed and found: One child's medication consent was not signed by the parent. One child's record was missing their physical description or picture.
Although attendance was accurate, the BUTTERFLIES did not bring this maintained attendance with them when they left the classroom to walk down the hall to use the bathrooms.
The following was observed to be accessible to children: In FIREFLIES, the air conditioner cord was loosely hanging next to the pole and the sink is not working and in BUTTERFLIES, two low bookshelves and the lateral bookshelf are not secure and tippable.
The Building Inspection is current and expires 8/1/2025. The Fire Inspection is not current and expired December 2024.
One child with a (REDACTED) and medication was not listed on the required posting.
All IHCPs were reviewed and it was found one IHCP was missing for a child with (REDACTED) and one child's IHCP was missing the names of the educators trained to administer the medication.
The thermometer was not in the refrigerator.
The push down/gate locking mechanism on the outdoor fence surrounding the playground is stuck and does not work. This is a safety hazard as the gate cannot be properly closed to keep children with in the fence.
The evacuation drill log was reviewed and found drill were not documented in October or November 2023.
