Previously cited on 2/15/2024 (#142836): "Two sets of bathrooms were observed during this visit. The bathrooms near the library both had hazards. The boys' bathroom had a lot of rust along the bottom of the wall partitions between the toilets. In the girls' bathroom, the wall heater had a piece of hot pipe exposed on the left side." Corrective Action Plan submitted on 2/26/2024: "We will share this information with the school maintenance department and follow up with them about a plan to address the hazards." Visit on 2/13/2025: Two sets of bathrooms were observed during this visit. The bathrooms near the library both had hazards. The boys' bathroom continues to have a lot of rust along the bottom of the wall partitions between the toilets and the girls bathroom has exposed wires on the outer wall near the windows (possibly a sensor that has a missing cover).

Six child records were reviewed and found: One child's record had an incomplete IHCP (missing consequences) and parents did not date the medication consent form. One child's record did not have a current medication consent form (dated 1/14/2024) and the IHCP was not dated by parent. One child's record had an incomplete IHCP not (missing consequences).
Three staff records were reviewed and found: One staff record was missing a physical and evidence of immunity to MMR and one staff record does not have a current physical (last dated 7/29/2022) and no evidence of immunity to MMR.

Children were observed to eat snack without washing their hands first. The licensee must ensure that educators and children wash their hands with liquid soap and running water, using friction, in accordance with Department of Public Health guidelines. Hands must be dried with individual or disposable towels or automatic hand blow-dryers. The use of common towels is prohibited. Educators and children must wash their hands at least at the following times: 1. before and after water play; 2. before and after eating or handling food; 3. after toileting or diapering; 4. after coming into contact with bodily fluids or discharges (including sneezes, coughing); 5. after handling animals or their equipment.
All IHCPs were reviewed for children with chronic medical conditions. One IHCP was incomplete. It was missing who is trained to administer and what the side effects of the treatment are and the same IHCP listed two medical conditions. Each medical condition must have its own IHCP.
