The evacuation drill log was reviewed and found no evidence of a drill conducted in January 2026. Drills are to be conducted every month a program is operating.

Three child records were reviewed and found all were missing developmental history.
Four children were in attendance during the visit and none of their water bottles were labeled with their names. Personal items intended for individual use by children, including but not limited to bottles, pacifiers, toothbrushes and sleeping materials, must be labeled with the name of the child for whom they are intended.
Three child records were reviewed and found two children with chronic health conditions were missing the doctor allergy action plan.
The following was observed during the visit and will be accessible to children: the air purifier on the ledge between the kitchen and classroom can slide off the ledge and could pose as a hazard if it drops on a child or adult.
The following was observed during the visit and will be accessible to children: all of the bolts on the fence gates and corners of the fencing are longer than three threads. These long bolts can be a catchment hazard for clothing.
The followed was observed during the visit and would be accessible to children: The space between the rungs on the outdoor side stairs and landing are approximately 3.66 inches wide (entrapment is space between 3.5 and 9 inches).
