A review of the staff records indicated the following: -The licensee did not perform bi-monthly observations on the educators. The licensee must describe in writing and implement its plan for regular, ongoing supervision of all educators, as appropriate to their positions. Supervision must include, but not be limited to: 1. observation of educators while working with children at least every two months by a staff with lead teacher, site coordinator or higher qualifications; 2. consultation with educators regarding children’s individual needs and communication with families; and 3. documentation of all observations and consultations.

The Toddler attendance had documentation that 4 toddlers were present. The toddler classroom actually had 9 toddlers present. Programs must maintain accurate attendance, staff child ratio, and appropriate group size.
In the toddler classroom backpacks, pillows, and blankets were being stored in a crib. Cribs should not be used for storage in a classroom.
The infant classroom did not have emergency/evacuation procedures posted at all exits.

A (REDACTED)was observed sleeping in a sleeping bag with an empty bottle still in their mouth. No Bottles. Bottles must never be propped, and babies should not suck on a bottle while sleeping. Propping the bottle increases the risk of choking and of ear infections. Falling asleep with milk pooled in the mouth leads to serious dental caries in developing teeth.
A review of eight staff records indicated the following: -Eight did not have a completed staff information form. -Eight did not have a completed EEC orientation checklist form. -Five did not have a current physical examination. -Six did not have documentation of being fully immunized according the Department of Public Health. -Two did not have documentation of an interview for the current position. -One did not register annually with the Department of Early Education and Care.

A review of nine children's records indicated the following: -All nine children did not have a current authorization and consent form completed. -All nine children did not have a current transportation plan. -One did not have a current physical examination.
Two preschool children were missing from the attendance during the visit on 4/26/22. It took the Director approximately 5 minutes to figure out who was present. Programs maintain accurate attendance for all children present at the program daily.
Two toddlers were playing with rubber gloves in the classroom. Rubber gloves are hazards and should be inaccessible to children.
