BRCs on all potential employees
7 children's records were randomly selected for review, and the following was learned: 1) 4 children did not have evidence of a current face sheet, first aid/emergency consent form, transportation form, developmental history form or off-site consent form as required by regulation. 2) Infant Child A started in the program on 10/27/2025, through documentation review, it was learned that their parents did not provide any of the EEC required paperwork until 12/7/2025, creating a potential hazard 3) the infant educators are required to review the developmental history forms of each child prior to directly caring for them, during the visit, it was learned that this is not consistently happening in the program as required, creating a potential hazard to the infants in their care.

In a review of the "preschool" classroom, it was observed that a "power strip" (for additionally outlets) near the diaper table.1) Not all empty outlets had covers as required 2) the cords coming out of the power strip were not secured as required, creating a potential hazard to children.
The following was observed during a review of all indoor licensed space: 1) Adult scissors were in the reach of children in the preschool room, creating a potential hazard 2) chipping paint was observed on the vent in one of the indoor gym space, creating a hazard 3) a hot coffee cup was observed in the reach of children when it was left on a low shelf in the "pre-K" room, creating a potential danger. The program was previously cited for an accessible hot coffee cup during the 11/1/2024 visit.

During a review of 7 children's records, the following was learned: 1) 2 children did not have written evidence of a current progress report as required.
During a review of all staff BRC, the following was learned: 1) Educator A (start date 2015) did not have documentation to support that they had a current BRC as required by regulation (expired 12/2024)
During an overall review of the program's indoor licensed space, it was learned that in all five ECE Classrooms, and in the indoor gym spaces, the majority of the equipment, including toy shelves and kitchen sets were unsecured, creating a potential hazard to children.
During a review of the licensed outdoor space, the following was observed: 1) bolts are pulling out of the railroad ties in the school age playground, creating a potential hazard 2) a garden hose was left on the ground on the preschool playground, creating a tripping hazard.
Chipped paint was observed throughout the early childhood space, creating a potential hazard to children.

During an observation in the "preschool B" classroom, the educator did not use positive child guidance as required by regulation.
During a review of the program's indoor space, the following was observed: 1) not all equipment, including toy shelves were secured as required by regulation, creating a potential hazard 2) Evacuation maps and instructions were not observed to be posted in each classroom as required 3) In the preschool B classroom the educator's hot coffee cup was sitting in reach of children on the "teacher's desk" creating a potential hazard
During a review of the outdoor playground space, the following was observed: 1) the bolts on the railroad ties on the school aged playground were observed to be pulling up, creating a potential hazard.

Not all shelves were secured as required by regulation.
During a review of a diaper change in the "infant/toddler" room it was observed that the educator did not keep one hand on the child while changing on an evaluated surface as required. The Educator was then observed to use only one step to clean/disinfect the changing table, a 2-step process is required by regulation.

The following was observed during a health and safety walk through of the program: 1) not all shelves, kitchen sets, and equipment were observed to be secured as required 2) throughout the program, unsecured cords were observed, creating a tripping hazard 3) chipped paint was observed throughout the program, creating a potential hazard 4) Several closets were observed to be unsecured creating a potential hazard.

During a random selection of three staff files, it was learned that 2 staff have not provided evidence of current physicals as required by regulation.
On the day of the visit, it was observed that there were several sections of chipping paint in the upstairs section of the program, posing a potential danger to children.

During a review of the outdoor space, it was observed that the green temporary fencing that serves to minimize an entrapment hazard is pulling away in sections and was observed to have a broken section, creating a potential hazard.
