The program recently starting keeping electronic files for all staff. During the visit, the Director was not able to locate some of the documents/information for several of the staff at the program, including most recent physicals and certificates/qualifications. Please review all staff records and ensure they are all complete and up to date.
A written progress report must be done periodically
In the Blue Room, Red Room, and Green Room (extended day), the exit sign and exit procedures were only posted at 1 exit, not the 2nd exit.

Medication Storage; Medication Container/Label
Current consent for administered medications
It appears as though Progress Reports were completed for all of the enrolled children in the fall. However, there was no date on any of these reports, so Licensors were not able to verify the date they were completed.
In the Red Room, the program has Budesonide for a child, and there is no prescription on the medication or the container.
There is a child in the Red Room who is prescribed [REDACTED] and the medication consent form is for [REDACTED] However, the program does not have [REDACTED]for the child. Instead, the medication at the program is [REDACTED] The same child has [REDACTED] at the program. However, there is no medication consent form for the [REDACTED], and the IHCP does not have [REDACTED]listed as one of the child's prescriptions.

1) Several stained ceiling tiles were observed in program classrooms, creating a potential hazard to children 2) In "Red" the dramatic play set was unsecured, creating a potential hazard 3) directly between the dramatic play set was chipped/ripped wall board, creating a potential hazard.
During a review of Child A's [REDACTED] medication, the following was learned: 1) The [REDACTED]11/2024 2) The [REDACTED] prescription on the box expired 11/2024 3) Child A had an IHCP that required [REDACTED] Child A had "chewable Benadryl" on site, however a review of the box stated that children between 2-5 should not use unless directly by the doctor, who ordered [REDACTED]

Indoor Space
During a review of the program's licensed indoor space, the following was observed: 1) In the "orange" room there was a shelf that is on wheels and when the shelf is pulled out, there is a nail protruding out of the back of the shelf, creating a potential hazard 2) There was an observed stained ceiling tile in the "Green" room creating a potential hazard.
Three Children's records were randomly selected for review and the following was learned: 1) One child did not have evidence of having provided the program a current physical as required (last physical on record 12/6/2021).

Indoor Space
Two staff records were randomly selected for review and the following was learned: 1) Educator A did not have evidence of a current first aid certification as required. 2) 2 Educators did not have evidence of having completed the EEC required "Strong Start" trainings as required. 3) Educator B did not have evidence of having provided the program with [REDACTED] records as required.
During a health and safety review of the program, the following was learned: 1) Stained ceiling tiles were observed in the "Yellow" classroom, creating a potential hazard to children.
During a review of the outdoor space, it was observed that the entrance gate access was uneven, creating a potential entrapment hazard.

In the "blue" room and in the lobby space, there was an outlet missing a cover. The closet space in the "blue" room was not secured and educator's purses were observed to be stored there, creating a potential risk to children.
